Job Title: Registered Nurse (Part-Time) - Bay Creek Dialysis Location: Bay Creek Dialysis - 2010 Empire Blvd Webster, NY 14580 Hours Per Week: 20 Schedule: Site open 5a - 930p. Shifts will rotate during these hours D/E Monday-Saturday (every other weekend requirement) OUR Department: Rochester Regional Health offers a full range of Dialysis treatment options to patients with chronic kidney disease. We treat adults from people from multiple backgrounds, cultures and ethnicities. We care for patients temporarily, either their kidney function recovers or they return to their home dialysis center, move or get a transplant; or some are with us on an ongoing basis. Patients have dialysis treatment anywhere from 2-3 days a week, from 3 to 5 hours each treatment. Our staffing model consists of RN’s, LPN’s and Techs. SUMMARY: As a Registered Nurse (RN), you’re the engine and the heart of patient care at Rochester Regional Health. You make a positive difference every single day in the lives of our patients, their family members and visitors – going above and beyond to meet their needs with courtesy, kindness, respect and compassion. RESPONSIBILITIES: • Performs initial and ongoing assessment of the patient, utilizing appropriate data (i.e. physical, psychological, educational and risk-screen data appropriate for the chronological / developmental age of the patient). • Demonstrates clinical judgment in responding to priority patient needs. • Communicates and documents assessment of patient with the interdisciplinary team. Utilizes all available resources in order to meet the needs of the patient and family. Planning: • Develops and documents plan of care, treatment and services which is individualized and appropriate for the patient needs, strengths, limitations and goals. • Includes the interdisciplinary health team as needed. • Demonstrates effective communication with the patient / family and interdisciplinary team. Implementation: • Provides care, treatment and services in collaboration with interdisciplinary health team. • Maintains an appropriate nurse-patient relationship. • Complies with organizational policies, procedures and protocols including policies governing safety, medication administration and documentation. • Initiates and documents education plan for the patient and family based on identified learning. Evaluations: • Evaluates, communicates and documents patient responses to care. • Revises plan of care based on ongoing evaluations and updates documentation appropriately. Education: • Ensures the patient and family receives / understands education / training specific to their needs and abilities using the teach back method for evaluation and documents appropriately. Demonstrates Compassion, Leadership and Professionalism: • Confidently leads the care delivery team through effective communication, collaboration and leadership. • Assumes responsibility and accountability on the unit / patient care team. • Demonstrates the ability to work collaboratively with others. • Demonstrates knowledge of legal aspects of nursing as evidenced by documentation and professional behaviors. • Delegates appropriate responsibilities to assigned nurses and ancillary staff. • Demonstrates ability to manage job related stressors and constructive feedback during intra/interdepartmental relations. • Demonstrates flexibility in meeting staffing needs throughout the organization. • Participates in problem-solving activities at the unit level by identifying the problem and collaboratively working toward resolution. • Provides input regarding protocols, policies and procedures to immediate supervisor. • Provides training / precepting for new team members on the unit. Performance Improvement / Evidence Based Practice / Research: • Demonstrates knowledge of evidence-based practice within practice area and identifies opportunities for performance improvement. • Participates and utilizes the performance improvement process to improve clinical practice. • Completes at least one clinical research utilization (CRU) form for submission to Nursing Research Department utilizing a research article from the last 5 years from research performed in the United States. Compliance • Completes a minimum of 5 nationally approved contact hours (CNE or CEU) per year. • Completes mandatory in-services and health screens in timely manner. • Maintains Basic Life Support (BLS) certification. REQUIRED QUAILFICATIONS • Diploma or Associate’s Degree in Nursing required; Bachelor’s Degree in Nursing preferred. • Registered Nurse license in New York State. • Basic Life Support (BLS) certification PREFERRED QUALIFICATIONS • For part-time or per diem positions within dialysis, prior dialysis experience is strongly preferred. This team is led by a critical care board certified intensivist faculty and its members include residents, fellows and advance practice providers. We are looking for APP partners who would maintain continuity of this high intensity cardiothoracic surgery critical care team. We are seeking a Nurse Practitioner or Physician Assistant to: Work with an interdisciplinary team to provide peri-operative care for adult patients undergoing cardiothoracic surgery Participate in daily rounding and collaborate with attending or consulting team to adjust plans of care. Performs a variety of clinical duties which promote a safe and organized environment for the delivery of services to children and their families within the urgent care setting. This position reports to an Urgent Care supervisor. Essential Functions Adheres to all Children’s Wisconsin and Urgent Care policies, procedures, and standards. Ensures confidentiality to our customers by creating and maintaining a secure and trusting environment; maintains HIPAA compliance. Provides exceptional care to patients and families to fulfill our patient experience promise. Regularly performs RN duties and maintains own patient care competencies as determined by position scope of practice. Notifies provider of any patient needing immediate assistance. Administers medications; performs point of care testing according to established policies; collects and labels specimens per policy; performs procedures as appropriate; documents accurately in chart; assists with narcotic counts; stock supplies to maintain efficient clinic flow. May delegate clinical tasks as appropriate. Performs clinical assessments within scope of practice, including triage. Effectively prioritizes and categorizes patients based on needs and acuity. Attends staff meetings and in-services, and keeps abreast of communication that affects patient care, services, and clinic operations. Provides education and discharge teaching per scope of practice. Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures. Age-specific information is developed further in the departmental job standards. Education: Graduate of an accredited school of nursing in order to meet the recommended entry level for a professional nurse required Bachelor's Degree BSN preferred Experience: 2+ years experience in a primary care or urgent care setting preferred Experience with pediatric patients preferred Knowledge, Skills and Abilities Ability to work with a diverse patient population to promote a culturally competent environment. Travel to all Urgent Care locations required. Ability to work daytime, evenings, weekend, and holiday hours. Electronic Health Record proficiency preferred. Excellent organizational and prioritizing skills necessary to work independently in a fast paced multi-functioned department. Requires excellent interpersonal and communication skills in order to assist patients, parents, and providers, and other staff. Analytical skills are necessary to assess complex patient conditions effectively. Requires the ability to synthesize multiple sources of information to assist in patient care. Required flexibility in order to prioritize efforts to meet needs of the patients, parents, providers, and other staff. During periods of high volume, additional shifts may be required. Nurse Practitioner / Physician Assistant – Inpatient Orthopedics

Work Schedule: 100% FTE, four 10 hour shifts, Monday – Friday between the hours of 7:00 AM – 5:00 PM. No holidays or call. May require occasional five 8 hour shifts based on staffing needs. We are seeking a Nurse Practitioner or Physician Assistant to: Provide comprehensive care to orthopedic patients in an inpatient setting. Conduct comprehensive or episodic health history and physical assessment of patients/families with acute health problems. Education: If Nurse Practitioner: Master’s or Doctorate’s degree from accredited Nursing Program. If Physician Assistant: Bachelor’s or Master’s degree from accredited PA Program. Licenses and Certifications: Minimum – State of Wisconsin NP or PA licensure If Nurse Practitioner*: Active RN and APRN certifications and licenses, including prescriptive authority, in state(s) of practice AND Active board certification as a NP in area of clinical practice.
