Identifies risk levels, precautions, and expected outcomes individualized to the client to include discharge planning; prescribes interventions to attain expected outcomes; evaluates the effectiveness of interventions in relation to expected outcomes; conducts ongoing assessment of clients from admission to discharge; initiates medical/psychiatric crisis intervention to stabilize and ensure the well-being of the patient's conditions. Demonstrates knowledge of psychiatric disorders and appropriate nursing interventions; implements nursing interventions identified in the plan of care that is individualized to the patient needs. Applies appropriate interventions to progress the patient towards wellness and addresses maintenance of wellness. Administers medications as prescribed safely; provides education on prescribed medications; recognizes signs and symptoms of adverse drug reactions. Demonstrates professionalism, accountability, and responsibility for decision making, quality nursing care, and knowledge base of psychiatric nursing care. Reviews, revises, and updates medical records within facility guidelines and timeframes. Transcribes and implements physician's orders according to facility policy; documents care, progress, response to intervention in medical record in an accurate, thorough and concise manner. Follows up on client care responsibilities, communicates incomplete assignments, and current client needs to staff to assure continuity of care. Job Title/Position : Hospice On Call RN Reports To : Clinical Director JOB DESCRIPTION SUMMARY : The On-Call Hospice RN is responsible for responding to all of the Hospice patient census and their families, as well as handling any referrals or community inquiries outside of office hours (nights, weekends and holidays). The on-call nurse will provide urgent clinical and psychosocial interventions for patients and their families, skilled pain and symptom management and collaboration with providers within and outside of the hospice team to maximize positive patient outcomes. The work of the On-Call Hospice RN provides considerable consideration for independent assessment and judgement in crisis situations, utilizing and consulting with the Hospice Medical Director and administrator on call as indicated. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES : Consistent availability by telephone and email during assigned shifts. These areas serve outpatient and inpatient patients who require procedures that require medical imaging to help diagnosis and treat certain problems. They see infants and adults. There are two sides to this. One group of nurses prepare and recover the patient following the procedure, while another group of nurses is in the rooms with the interventional radiologists and others to assist with the procedures. Some of these nurses do go over to MD Anderson for Radiologic cases scheduled over there. Job Description Nemours is seeking a full-time (60hrs every 2 wks) Nursing Assistant for our 4 East Unit, with every third weekend, holiday rotation package and flex holiday (as needed) in Wilmington, DE. These computers are also used in the barcoding administration of medications which ensures patient safety by verifying that the correct medication is given to the appropriate child. This unit’s goals are to provide high-quality, cost-effective, family-centered care. Family-centered care rounds are performed daily to assure an inter-professional approach and partnership with the patient and family. Essential Functions Ensures an adequate stock of all necessary equipment and supplies in designated areas at established par levels. Ensures a clean and safe patient care unit. Assists patients with activities of daily life (bath, feed, play, etc.) and provides other patient care as directed. Assures all supplies are accurately charged. Contacts pharmacy, dietary, and other departments as designated by the Registered Nurse. Collects patient specimens (blood, urine, stool, other per policy), paying close attention to patient safety, comfort and accuracy of information. Uses Hospital Information Systems as required for patient care, (e.g. documenting vital signs and patient care activities.) Accurately relays information from physicians, nurses, laboratory personnel and others. Transports patients and delivers reports, supplies, etc., to other patient units or Institute locations as needed. Courteously and professionally covers patient care unit front desk as needed. Job Description The PICU is seeking a Registered Nurse to join our team in Wilmington, DE. This is a 36hrs/wk, rotating shift position. The Pediatric Intensive Care Unit of Nemours Children's Health, Delaware is a 24-bed unit in a Level 1 Trauma Center. Patients requiring care and surveillance in the PICU will have conditions and diagnoses which may include, but are not limited to, those producing: acute and reversible loss of consciousness, respiratory failure, compromised air exchange, cardiovascular dysfunction, hepatic failure, hemodynamic instability, renal dysfunction, central nervous system injury or dysfunction, acute trauma and organ failure requiring transplantation. Treatments or interventions performed in the PICU include measures utilizing advanced life support technology. These include, but are not limited to cardiopulmonary resuscitation, tracheal intubation, ECMO, pharmacologic support of circulation, mechanical ventilation, point of care monitoring, CRRT, thoracentesis, and hemodynamic, intracranial pressure, intravascular monitoring. Each patient room has electrocardiographic, pulse oximetry, respiratory, hemodynamic and intracranial pressure monitoring capability, as well as central EKG and respiratory monitoring. Portable monitoring during patient transport is provided. Special equipment includes radiant warmer beds, heating/cooling blanket systems, Mapleson manual ventilation system, end tidal CO2 monitors, ventilators, hemodynamic monitoring systems, intracranial pressure monitoring system, ventriculostomy systems, infusion pumps, blood warmer, continuous renal replacement systems, and peripheral nerve stimulators. Requirements Be a graduate of an accredited school of nursing; BSN preferred. Experienced pediatric intensive care nurse of at least 2 years preferred. Current Delaware or Multistate Compact License Must be CPR certified by the American Heart Association. PALS and ACLS preferred.
