Overview JOB DESCRIPTION – POSITION REQUIREMENTS JOB TITLE: Staff Nurse II CC/Department: 7010/Emergency Services Reports to: Administrative Director of Emergency Services JOB SUMMARY: The Staff Nurse is a professional caregiver who assumes responsibility and accountability for the assessment, nursing diagnosis, planning, implementation and evaluation of care for patients and their families in compliance with the standards for care at Seton Medical Center and Nurse Practice Act. Performs ongoing assessment as indicated. 2 Plans nursing care, based on priority of physical and psychosocial-emotional needs. a) Develops realistic short-term goals for each problem, involving the patient, family or significant others. b) Plan demonstrates understanding of age-related needs of the patient. c) Negotiates with patient, family, significant other and health care team members in planning for education and discharge needs. 3 Implements the plan of care including prescribed procedures and treatments. Organizes and prioritizes care based on individual needs and unit activity. Assesses for changes in patient’s condition and appropriateness of the plan. Initiates interventions using appropriate clinical expertise and knowledge. As Primary Nurse, coordinates the activities of health care team members in completing diagnostic and therapeutic interventions. 4 Evaluates the effectiveness of nursing care delivered to reach the expected outcome. a) Evaluates the plan of care according to the current condition and changing needs of the patient. b) Accurately and concisely documents the patient’s status and response to the care delivered. Issues related to assault or abuse. 7 Analyzes patient related data, such as vital signs, past medical history, physical exam findings, laboratory values and other information, including age-specific criteria, to determine appropriate care. 8 Utilizes advanced, specialized knowledge and skills to implement interventions for all Emergency Services patients, by prioritizing needs, and using a patient centered approach. 9 Provides health education and teaching to patient’s, family members and/or significant others, that is age specific, providing for any barriers to learning. Clearly communicates health, treatment and discharge information, ensuring the patient, family member or significant other understands all discharge and follow-up instructions. 10 Effectively performs procedures, such as initiation of IV catheters and fluids, medication administration, dressings, patient teaching, etc., within their scope of practice, and unit, department and Medical Center policies and procedures. 11 Effectively performs other nursing procedures/skills, such as cardiac monitoring, rhythm interpretation, defibrillation, monitoring of conscious sedation, ABGs etc., based completed specialty training and their scope of practice, and unit, department and Medical Center policies and procedures. 12 Appropriately recognizes and quickly responds to emergency medical situations. a) Accurately assesses “A-B-Cs.” b) Initiates appropriate emergency treatment, i.e., CPR, O2, etc. c) Communicates critical information to other team members. 13 Maintains all required credentials and training (CA RN license, BLS, ACLS, PALS, NIHSS, AB508). 14 Remains current on emergency treatments/practices/procedures through attendance at staff meetings, continuing education events, in-services and the review of professional literature. 15 Integrates/applies new knowledge from education and research into clinical practice. 16 Demonstrates knowledge of and competence in age-related patient care. 17 Consistently demonstrates knowledge of state and federal regulations, such as Title XXII and COBRA/EMTALA. Description Kindred Hospital San Francisco Bay Area is a 99-bed long-term acute care hospital offering the same in-depth care you would receive in a traditional hospital, but for an extended recovery period. We partner with your physician and offer 24-hour clinical care seven days a week so you can start your journey to wellness. For those who need special care, we offer a 10-bed ICu and two negative pressure rooms. We are located about 7 miles east of Oakland International Airport right off MacArthur Freeway (I-580). Job Summary Assists professional nursing personnel in providing patient care in assigned area. Assists patients with activities of daily living, provides for personal care, emotional support and performs more complex clinical skills under the direction of professional nursing personnel. Essential Functions Consults with and keeps supervisor informed of unit activities, needs, and problems related to patient care. Provides general nursing care such as positioning the patient, lifting, turning, applying/utilizing special equipment, assisting in the use of bed pan, urinal or commode, ambulating the patient, pre operative and post operative care, and application of binder or anti embolic stockings, bed bath, oral/hair/back care, changing bed linens, cleaning over bed table, and bedside stand, straightening patient room and other general care necessary during the shift. Takes and records temperature, pulse, respiration, weight, height, blood pressure, and intake and output measurements accurately. Documents in a timely manner. Prepares patient room and bed for admission and transfers and ensures all necessary equipment is in room. Maintains an attractive and comfortable environment for patients with special consideration to cleanliness of room, ventilation, and lighting. Assists in patient admission, transfer, and discharge procedures. Assist with handling and care of patient belongings and other personal property. Answers patient call lights, telephone, and paging system and responds as appropriate. Under the direction of the Registered Nurse, performs routine treatment procedures such as enemas, application of appliances for heat and cold. Able to release, remove, and reapply restraints to patient under the direction of an RN. Ensures patient is in proper position and alignment. Assist Registered Nurse and/or LVN/LPN with complicated patient treatment procedures. Serves and removes patient meal tray in timely manner. By joining our team you will be providing skilled services to help patients and families cope with the End of Life process. Schedule: Full Time, Part Time or Per Diem Responsibilities: Completes an initial, comprehensive and ongoing assessments of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es). Provides professional nursing care by utilizing all elements of nursing process. Write and initiate plan of care Regularly re-evaluates patient and family/caregiver needs Participates in revising the plan of care as necessary Use health assessment data to determine nursing diagnosis Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process. Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care. Provides health care instructions to the patient as appropriate per assessment and plan. Assists the patient with the activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optional comfort care. Acts as Case Manager when assigned by Clinical Supervisor and assumes responsibility to coordinate patient care for assigned caseload. Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given. Records pain/symptom management changes/outcomes as appropriate. Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required. Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers. Works in concert with the interdisciplinary group. As well as customized care and patient education about disease management, the Home Health RNs provide a variety of specialized services, such as Simple to Complex Wound Therapy, Post-Surgical Care Management, and Ostomy Care Management. We provide an entire internal operational support team for our clinicians to handle the tasks of pre-populating the medication reconciliation, tracking down DME orders, getting MD signatures, coding, lab follow-up, etc., truly allowing you to function independently, with the flexibility to make your own schedule and plan patient visits around other commitments if needed. The home care setting allows you quality time with your patient in the comfort of their own home.
