Provides privacy for residents and adheres to HIPAA standards. 7. Inquires about residents/guests comfort/discomfort level on a scale of 0-10. 8. Provides restorative care as indicated. 9. Performs routine catheter care 10. Provides care to residents/guests with tubes and special equipment. 11. Transports resident/guest to and from activities. 12. Answers all call bell and places them in reach of resident/guest. 13. Fills water pitchers with fresh ice water unless contraindicated. 14. Offers snacks to all residents/guests and documents appropriately. 15. Monitors residents during smoking periods when assigned. 16. Demonstrates ability to deescalate resident through verbal strategies, redirections and crisis management and report to nurse immediately. 17. Assist with meal pass and feeding in a timely manner. 18. Follows Fall Prevention Protocol. 19. Collects lab specimens. 20. Reports condition changes/behavioral issues to nurse immediately. 21. Follows elopement risk protocol. 22. Maintains cleanliness and organization of resident/guest rooms including bed making. 23. Accompanies resident/guests during outside medical consultations and out of building activities. 24. Follows safe-handling procedures for personal safety and resident safety at all times. 25. JOB SUMMARY The nursing assistant is an integral member of the health care team who delivers quality care through collaboration and communication with team members. They work under the direct supervision of the registered nurse to support and assist the nurse in the delivery of quality care to patients and families. Some of the major responsibilities will include collection and reporting of patient data, incorporating organizational competencies of superior quality care and assuming responsibility and accountability for their role performance outcomes while supporting GSPP's Standards of Excellence. ESSENTIAL FUNCTIONS PATIENT / CUSTOMER Essential Accountabilities Report Receives written/verbal report at the beginning of each shift from the registered nurse (RN) regarding the care that is to be delivered to the assigned group of patients, or for patient needing constant observation. Gives report when leaving unit, and at the completion of the shift regarding the care that was delivered; any abnormalities that occur during the shift are to be reported as they occur to the RN as well as to the oncoming CNA/NA. Assists in Assessment Identifies the patient per hospital policy before carrying out functions. Assists with data collection for nursing history and physical e.g. vital signs, ht. wt, blood sugar results, functional status and communicates changes to the RN in a timely fashion. Participates in respectful communication with patients, families and coworkers. Provides patient and family information to the registered nurse for coordination of care and discharge planning. Observes skin changes and reports discoloration, abrasions, infestations or skin breakdown. Planning Assists in setting mutually agreed upon realistic individual patient goals. Integrates the RN’s directions re: the patient’s plan of care into the care given to the patient. Gives RN report before the end of the shift regarding the patient’s response to the plan of care issues, and at various times during the shift to report unusual findings e.g. abnormal blood sugar results, vital sign changes, pain etc. Intervention Promotes infection control by using the appropriate hand washing techniques. Follows standard precautions and any other isolation precautions as dictated by the patient’s condition. Maintains a safe environment and protects the patient from injury by observing the patient who may be at risk for falls, confusion, disorientation, wandering or suicidal behaviors; follows Constant Observation Policy. Applies and monitors patients in restraints per policy and per medical order. Observes or monitors behavior/health state and responses to therapy. Evaluation Assists in evaluating nursing care given by comparing the patient's functional status from day to day and discussing this with the nurse. Education High School Diploma required Must be a nursing student matriculated in an accredited BSN nursing program having completed the nursing foundations/fundamentals clinical course with a 3.0 GPA overall and in the nursing major and continue to maintain a 3.0 GPA in all nursing major subjects. In order for nursing students to maintain their position as a NA, they must continue in a matriculated BSN program and present evidence with an official transcript documenting a 3.0 GPA overall and in the nursing major each semester to the Nurse Manager. Nursing Students are required to submit grade reports/transcripts to the Nurse Manager within 60 days of their course completion. The Nurse Manager will subsequently provide this information to Human Resources. Nursing students employed as NAs, must immediately (within 24 hours) report to their Nurse Manager (in writing) if they have not sustained the academic GPA requirement or discontinued/withdrawn from their matriculated BSN program. Failure to meet and maintain nursing student requirements in lieu of certification will result in termination of the nursing assistant position. If the Nursing Assistant completes a nursing degree and obtains nursing licensure they may continue to practice within the scope of the Nursing Assistant job description until they are able to transition into a different position within the organization. Assesses and evaluates patient’s status by: A. Writing and initiating plan of care B. Regularly re-evaluating patient and family/caregiver needs C. Participating in revising the plan of care as necessary 4. Initiates the plan of care and makes necessary revisions as patient status and needs change. 5. Uses health assessment data to determine nursing diagnosis. 6. Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process. 7. Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care. 8. Counsels the patient and family in meeting nursing and related needs. 9. Reports suspected abandonment, abuse, financial exploitation, or neglect of a person in violation of the department of social and health services and the proper law enforcement agency. Reports must be submitted immediately when the reporting person has reasonable cause to believe that abandonment, abuse, financial exploitation, or neglect of a vulnerable adult has occurred. 6. Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers. Works in concert with the interdisciplinary group. 7. Provides and maintains a safe environment for the patient. 8. Assists the patient and family/caregiver and other team members in providing continuity of care. 9. Works in cooperation with the family/caregiver and hospice interdisciplinary group to meet the emotional needs of the patient and family/caregiver. 10. Attends interdisciplinary group meetings.
