Job Duties: Works collaboratively with the providers to assess the needs of each patient on an individual basis and provide the services necessary to meet these needs. Completes admission assessment according to unit policy, utilizing past medical history, therapeutic communication and physical assessment. Identifies accurately, nursing diagnosis/collaborative problems to determine appropriate interventions within the framework of accepted standards of care. Assumes patient care responsibilities by organizing workload effectively and remaining flexible to environmental changes. Communicates professionally and effectively with patients, families, co-workers, healthcare team members and ancillary departments to ensure “Patient-Centered” coordination of care. Performs assessment and maintenance of central lines, PICC lines, and Port-a-caths, Groshong catheters, etc. including flushes and blood draws per established protocols. Participates in performance improvement activities to evaluate and improve patient care. Promotes an environment that encourages individual growth and heightens professional practice. Participates in professional growth and self-development activities to ensure accountability for own professional nursing practice. Participates in staff meetings, committees and special projects; actively contributes to the needs of Grand Itasca Outpatient Care Services and Grand Itasca Clinic and Hospital. Performs and assists with patient care, assessment and education as needed, including but not limited to: Performs IV Therapy including fluid infusions, antibiotic infusions, blood products, albumin, and medication infusions per protocol. Coordinates and administers chemotherapy to patients via oral, IV push, IV infusion, or bladder installation according to safety standards developed by the Oncology Nursing Society and the American Society of Clinical Oncology. Performs assessment and maintenance of central lines, PICC lines, and Port-a-caths, Groshong catheters, etc. including flushes and blood draws per established protocols. Attends and completes required training modules, in-services, and continuing education to maintain competency and professional licensure for demonstrated knowledge regarding the care and management of patients in the home and/or alternate care settings. Effectively provides oversight and coordination of paraprofessionals in the home setting and may act as the Supervisor in the absence of the Supervisor or Nurse Manager as applicable in accordance with state and federal regulations. Participates in multidisciplinary team conferences and provides precepting, training, and mentoring to other nurses for orientation, and onboarding and supervisory activities as assigned. Participates in nursing department on-call responsibilities. Performs other related duties as directed by supervisor. If applicable: Inserts PICC/Midlines per physician order. Troubleshoots/declots obstructed central line catheters. Monitors PICC/Midline and educates on prevention of line infection and routine care.
