Description The (Per Diem) Clinical Nurse is a Registered Professional Nurse who provides safe, competent quality care based on nursing theory and research to a designated group of patients and significant others. Responsibilities Patient Care Delivers patient care by incorporating the tenets of the professional practice model of Relationship Centered Care and through the application of the nursing process based upon theory, research, evidence-based practice, and approved organizational nursing standards. Assesses and evaluates patient care needs and applies critical thinking skills in patient care management. Integrates relevant assessment and intervention skills in the delivery of nursing care. Collaborates with the interdisciplinary healthcare team in the development, implementation, and evaluation of the plan of care. Prioritizes all aspects of patient care including teaching, rounding, coaching, and planning after hospital care and delegating to others as appropriate. Educates patients and caregivers while anticipating needs and readiness to learn, about their plan of care, transition of care, promotion of health, and prevention of disease. Communicates effectively and professionally with patients, family and all members of the Interdisciplinary Patient Care Team. Manages assignments within the Care Delivery Model of Modified Primary Nursing and demonstrates allocation of material resources effectively. Patient Experience Role models the tenets of Mount Sinai Relationship Centered Care (MSHRCC) through facilitation of the following actions including, but not limited to, Admission Welcome, Bedside shift Report, HELP, AM/PM Care, 5 Minute Sit Down, Purposeful Hourly Rounding, Medication Review, Discharge Wrap Up, and MD/RN Unit Collaboration. Considers the patients values, preferences, cultural diversity, expressed needs and knowledge in all aspects of care. Establishes and maintains a therapeutic relationship with the patient and family. Uses evidence-based practices to increase understanding of patients perceptions of care. Initiates service recovery for patients and family members who have concerns and escalates to Clinical Nurse Manager/Assistant Nurse Manager/Nursing Shift Manager or Patient Relations as needed. Demonstrates caring, respect, compassion, empathy and active listening through dialogue, body language and actions. Supports staff use of evidence-based practices to increase understanding of patients perceptions of care. Conducts Purposeful Hourly Rounding (PHR) on assigned patients addressing the 4 Ps. Quality and Safety Practices safe and efficient patient-centered care, identifying and escalating barriers and opportunities for improvement. Demonstrates clinical skills and knowledge crucial to quality and safety in the patient population served. Implements process improvement strategies based on institutional, evidence-based ensure and procedures. Engages in formal and informal peer and institutional review processes. Collaborates with interdisciplinary teams to create and implement quality improvement projects, evidence-based practice activities and nursing research studies. Contributes to quality and safety practices and required compliance measures. Implements principles of high reliability to identify and avoid high risk behaviors to provide a safe environment for patients. Escalates potential safety hazards or gaps from best practice following institutional chain of command protocol. Utilizes institutional information technology by documenting nursing practice to support quality and performance improvement initiatives. Operations Performs charge nurse duties as assigned. Coordinates with the Clinical Coordinator/Clinical Nurse Manager/Assistant Nurse Manager/Nursing Shift Manager to ensure supplies are appropriately utilized and inventory is sufficient for patient care. Escalates material or staffing resource deficits to charge nurse, Clinical Nurse Manager/Assistant Nurse Manager or Nursing Shift Manager. Demonstrates patient focused and cost effective approaches to patient care in terms of equipment, staff, supplies and all other resources. Documents in an accurate and thorough manner in compliance with hospital, regulatory and legal requirements and standards of care. Advances clinical competence in nursing practice to progress from novice to expert. Contributes to the profession of nursing through participation in professional organizations. Qualifications Graduation from an accredited Nursing program. Bachelor’s Degree in Nursing. (BSN) Associate’s Degree in Nursing (ADN) RN who is matriculated in a BSN Program with a graduation date within the year. Any Associate’s Degree in Nursing (ADN) RN with Nursing experience must be approved by the CNO. Attends and completes required training modules, in-services, and continuing education to maintain competency and professional licensure for demonstrated knowledge regarding the care and management of patients in the home and/or alternate care settings. Effectively provides oversight and coordination of paraprofessionals in the home setting and may act as the Supervisor in the absence of the Supervisor or Nurse Manager as applicable in accordance with state and federal regulations. Participates in multidisciplinary team conferences and provides precepting, training, and mentoring to other nurses for orientation, and onboarding and supervisory activities as assigned. Participates in nursing department on-call responsibilities. Performs other related duties as directed by supervisor. If applicable: Inserts PICC/Midlines per physician order. Troubleshoots/declots obstructed central line catheters. Monitors PICC/Midline and educates on prevention of line infection and routine care. Minimum of 1 years of nursing experience. Current CPR certification required. Basic Qualifications Demonstrated competency in patient care standards required for safe delivery of services and infusion skill sets applicable to agency programs and service needs. Advanced certification and training as applicable. Required licensure to operate a motor vehicle in the state of practice with access to a vehicle for business travel with proof of liability insurance. Basic knowledge of computer operating systems and software applications with the abilityn self-evaluation for annual appraisal and jointly sets professional growth goals with nurse manager. to apply knowledge in the effective use of nursing technology tools to communicate and document care provided. Physical Demand Requirements Ability to lift up to 50 pounds with a maximum lifting of 75 pounds. Exerting up to 20 pounds of force occasionally, or up to 10 pounds of force frequently. Physical demands may involve walking, standing, crouching, kneeling, turning, pivoting, balancing, stooping, reaching overhead, grasping, pushing, pulling, lifting and carrying. Fine motor skills and visual acuity required by this job include ability to see up close and from a distance, color and peripheral vision, depth perception and the ability to adjust focus. Team members in this job classification have the likelihood of occupational exposure to blood, body fluids and other potentially infectious materials. Possible exposure to hazardous substances with possible effect on reproduction, injury from needles, other sharps, fumes, chemicals, humidity, cold, heat, adverse weather elements, animals, secondhand smoke/vape and unpredictable home environments. Safety requirements include closed toe-shoes, facemask, goggles, gown and/or gloves and functioning device for communication in both routine and emergency situations. Preferred Qualifications & Interests (PQIs) Bachelor of Science in Nursing as granted by an accredited school of nursing preferred. 1-2 years of previous infusion nursing experience preferred.
