About this Job: Unit Highlights The Discharge Hospitality Center (DHC) Licensed Practical Nurse (LPN) facilitates a safe, efficient, and patient-centered discharge process for patients awaiting departure from the hospital. The LPN provides individualized discharge education and reinforces instructions to ensure patients and caregivers understand medications, follow-up appointments, and post-discharge care.
