Takes and records residents blood pressure and vital signs, administers catheter care, administers IV therapy to residents as ordered within license limitations, and provides respiratory care, e.g. oxygen, nebulizer treatments. Charts nurse’s notes in an informative and descriptive manner that reflects the care provided as well as the resident’s response to the care. Ensures the resident’s environment is as free from accidents and hazards. Immediately corrects/addresses potential safety risks as prudent and warranted. Monitors and directs non-licensed ancillary services as deemed necessary to meet professional standards and maintain the highest practicable level of resident care. Communicate/report information to residents, resident families, physicians, guardians, governmental agencies, and facility personnel as required by facility policy and State/Federal guidelines. Ensures that all aspects of resident care plans are implemented and maintained. Make recommendations for care plan changes to the IDT per professional standards. Ensures compliance with infection control standards. Immediately corrects/addresses identified instances of non-compliance. Follows and adheres to company policies and procedures regarding HIPAA (Health Insurance Portability and Accountability) compliance. All other duties, as assigned. Licensed Practical (LPN) Nurse Qualifications and Skills: Excellent written and verbal communication skills in English. Ability to multi-task and remain calm under stress. Analytical and problem-solving skills. Understands and follows infection control standards, policies and procedures. Must be able to adapt to a flexible work schedule and occasional overnight travel. Ability to relate positively, effectively, and appropriately with residents, families, community members, volunteers and other facility staff. Licensed Practical Nurse (LPN) Education and Experience: Nursing Degree or diploma from an accredited College or University. Possess a current license to practice in the State as a Licensed Practical Nurse. 1 year of experience preferred. Long-Term care experience preferred. Physical Requirements: Walking, reaching, bending, lifting, extended sitting, grasping, fine hand coordination, pushing and pulling, ability to distinguish smells and temperatures, all with or without the aid of mechanical devices is required. Limited potential for exposure to environmental hazards. Understanding and adherence to company safety standards and protocols required. Place pharmacy orders, for and administer all newly prescribed medications and document. 8. Carry out direct contemporaneous charting in your shift. Chart detailed monthly evaluations of each Guest that reflect his/her condition and progress. 9. Responsible for all nursing care of assigned Guests while on duty. Must notify appropriate persons if there is any significant change in a Guest’s condition or any transfer to a hospital. 10. Ensure that Guest care plans are being followed and asses each Guest’s status in accord with their care plan. 11. Round with physicians when needed. 12. Administer or supervise all treatments prescribed by physicians including but not limited to pressure ulcer care, Foley catheter care, and hot and cold compounds. 13. Supervise serving of prescribed diets and fluids intake and immediately report any issues to the physician and DON/ADON. 14. Must be knowledgeable of individual care plans and support the care planning process by reporting specific information and observations of the Guest’s needs, preferences and report any behavioral changes. 15. Provide wound care when needed. 16. Completes medical records documenting care provided and other information in accordance with nursing policies while maintaining strict confidentiality. 17. Assure that established infection control and standard precaution practices are maintained when providing care. Follow established safety precautions when preforming tasks and using equipment and supplies. 18. Reports all hazardous conditions, damaged equipment and supply issues to appropriate persons. 19. Maintains the comfort, privacy and dignity of Guests/guests and interacts with them in a manner that displays warmth, respect and promotes a caring environment. 20.
