The LPN provides medication administration, treatments, supervision of CENAs performing ADLs. Collaborates with the Care Coordinators and other team members to ensure communication and timely updates on residents change in conditions. We do 12 hours shifts from 6:30am-7pm and 6:30pm-7am. Responsibilities KEY RESPONSIBILITIES INCLUDE: • Gathers assessment data, monitors comprehensive physiological, psychological, sociocultural, and spiritual needs of patients/significant others under the supervision of the Team Leader. Reports findings to the Team Leader. • Provides competent care consistent with the nursing process and written plan of care, physician’s orders and in accordance with skilled nursing facility policies and procedures. • Participates in admission and discharge planning, in collaboration with health care team, at the time of admission and continues through discharge. • Provides patient and family education to meet their specific needs as outlined in the plan of care and delegated by the Team Leader. • Documents care consistent with guidelines. • Performs other duties consistent with purpose of job as directed.

How you make a difference The Licensed Practical Nurse (LPN) plays a vital role in patient care by contributing to the nursing process of assessment, planning, implementation, and evaluation. Working under the supervision of RN(s), the LPN provides direction and guidance in patient education and activities that align with their education and demonstrated competencies. Key Responsibilities Under RN supervision, assesses patients' physical, psychological, and social dimensions in both the Health Care Unit and housing units, contributing to individualized treatment planning and consulting with RNs and staff as needed. Implements treatment programs directed by Health Care Practitioners, administering medications, including IV therapy (if certified), and performing diagnostic tests according to established protocols and procedures. Collaborates in controlled substance counts at the beginning of each shift, adhering to safety techniques, procedures, and standard precautions while responding to codes or health emergencies within standard guidelines. Assists in medical or minor surgical procedures, implements nutrition and therapeutic diet plans, and documents nursing encounters using SOAP charting, ensuring effective communication with nursing staff, physicians, and other relevant personnel. Additional Details We are looking for a full time LPN to join our clinical team at Macomb County Jail. This position works 6p-6:30a and rotates weekends. Qualifications & Requirements Education Graduate from an accredited school of nursing. Experience Prefer a minimum of one (1) year clinic experience Licenses/Certifications Have and maintain current licensure as a Licensed Practical/Licensed Vocational Nurse within the state of employment.
