Document patient information accurately in electronic health records (EHR). Educate patients on medications, chronic disease management, and preventive care. Monitor patient condition and report changes to supervising nurses or providers. Assess patient symptoms via phone or in‑person to determine urgency. Prioritize care using established triage protocols and assign urgency levels. Provide medical advice within LPN scope, including self‑care instructions for non‑urgent issues. Document all triage interactions and recommendations in the EHR. Coordinate referrals to physicians, specialists, urgent care, or emergency departments. Recognize changes in patient condition and escalate complex cases to RNs or physicians. Educate patients on medication management and preventive health measures.
