Responsibilities Work Schedule: Branch Clnics: Monday - Friday for a period of 8.5 hrs between the hours of 0645am and 5:00pm, with 30 minute lunch break. No On-Call, No Weekends, No Holidays!! Core Duties: Initiate, perform and complete assigned duties in providing care to variable patient populations. Complete assigned patient care based on the patients’ conditions, use judgment in selecting the appropriate order and sequence of procedures and treatments, and accurately recognize, report and record relevant patient information. Prepare and administer prescribed medications (oral, topical, subcutaneous, intramuscular and/or intravenous) and perform treatments according to established policies/procedures. Recognize urgent or emergent patient care situations, seek assistance of the RN and/or MD/DO, and initiate appropriate emergency interventions as directed. Perform support duties for complex diagnostic tests and/or specialized practices or procedures, which include preparing the patient, assisting in the diagnostic examination, preparing and handling specialized instruments or other specialized equipment, and monitoring the patient’s condition before, during, and following the procedure. Orient peers and ancillary personnel to unit policies and procedures, skills, and standards of practice. Integrate healthy work environment principles that result in improved patient healthcare delivery, education of patients and their families and employee satisfaction. Additional Duties vary dependent upon clinic/department assigned: Branch Clinics: Participate in MHP/MCMH Team huddles/meetings each day to review scheduled appointments, identify MHP/MCMH Team requirements (i.e., supplies and equipment and staff support issues related to the day’s scheduled appointments) and scheduled patient requirements (i.e., scrubbing the schedule to identify diagnostic procedures or medical readiness requirements that may be ordered or prepared before the patient appointment). Prepare patient electronic health record and other appropriate forms prior to patient’s appointment. Assist the MCMH team in managing many aspects of patient’s care: referrals, hospitalizations, ER visits, ancillary testing and other as directed to achieve optimum outcomes in safe and cost effective manner. Develop/distribute lists of community resources available to patients. Establish and maintains a relationship with consulting providers, hospitals, ERs and other frequently utilized healthcare resources to serve as a bridge across the continuum for the patient. Coordinate consult/referral/ follow-up/tracking process for the practice. Coordinate continuity of care with patient to insure compliance. Qualifications Minimum Qualifications: • Degree: Certificate • Education: Graduate from an accredited community college, junior college, college, university or vocational nursing program approved by the appropriate State agency and accredited by the Commission on Nursing Education Accreditation (CNEA) and HCW may be a graduate from a State Board of Nursing or accredited nursing program. • Certification: Possess a current AHA or ARC BLS Healthcare Provider certification • Experience: One year of practical or vocational nurse experience after graduation, and possess a minimum of 1 year full-time clinical experience within the last 2 years as a LPN/LVN. • Licensure: Current, full, active and unrestricted license as a practical or vocational nurse. * Security: Must possess ability to pass a Government background check/security clearance.

This work-from-home telephonic nurse role reports directly to the Director, Contact Center. The Clinical Care Coordinator is responsible for providing expert support in post-acute care navigation and serving as a knowledgeable resource on our services, including home health, hospice, and palliative care. The Clinical Care Coordinator ensures continuity of care for patients transitioning into or out of our services, determines appropriate levels of care, and educates patients and families on available post-acute care options. This position plays a critical role in connecting patients with the right services while ensuring a seamless, compassionate experience. The schedule is Saturday & Sunday 8pm to 8:30am Friday 12am to 8:30am Thursday 11am-7pm Key Responsibilities Serve as the main point of contact for patients and families transitioning from or seeking post-acute services. Make clinical level-of-care determinations using discussions, medical records, and other clinical data. Provide patient education and guidance on home health, hospice, palliative care, Medicare coverage, billing, and other healthcare resources. Act as a customer advocate throughout the referral process to ensure timely responses and maximize admissions conversion rates. Coordinate follow-up and track referral outcomes to ensure high-quality care transitions. Utilize multiple tools and methods to provide patient options and recommendations regarding post-acute care. Assist in the admissions process, supporting patients who meet requirements and ensuring satisfaction with services. Maintain awareness of departmental performance objectives and patient satisfaction goals. About You Qualifications Associate degree required; Bachelor’s degree preferred. Minimum of 3 years of post-acute care experience. Home health and hospice experience strongly preferred. Licensed Registered Nurse (preferred) or LPN with active professional licensure in a compact state (must be willing to obtain additional state licensure as required). Strong knowledge of home health, hospice, and palliative care services. Excellent communication, analytical, and customer service skills. Strong computer and technology skills; proficiency in Microsoft Office Suite.
