The LPN assists in assessing, planning, delivering, and documenting patient care, supports patient/family education, and contributes to the overall quality and safety of the healthcare environment. Care is provided in accordance with ACH standards, regulatory requirements, and within the scope of practice for LPNs. Responsibilities: Provide age-appropriate nursing care under the supervision of a Registered Nurse (RN) or designated provider, applying basic knowledge of biological, physical, behavioral, social, and nursing sciences to meet the needs of individuals and groups. Collects and monitors patient data, including packaging and obtaining specimens and labs, contributing to assessments and promptly communicating abnormal findings or changes in condition to the appropriate provider. Participates in developing and implementing patient care and discharge plans under RN direction, delivers nursing care and treatments as ordered, and assists with personal care and activities of daily living to maintain hygiene and nutrition. Safely administer prescribed medications, injections, and treatments while monitoring for adverse reactions, and ensure all providers orders are followed for patient treatment. Provides education to patients, families, and caregivers regarding health, treatments, and self-care. Document all aspects of patient care in accordance with hospital policies and nursing care standards. Adheres to regulatory standards and ACH policies, maintains a safe and clean environment, and reports safety concerns as appropriate. Works collaboratively as a member of the patient care team, engages in ongoing education and professional development, and performs additional duties to support efficient operations and quality patient care. Drug Administration Functions Prepare and administer medications as ordered by the physician. Verify the identity of the resident before administering the medication/treatment. Ensure that prescribed medication for one resident is not administered to another. Ensure that an adequate supply of floor stock medications, supplies, and equipment is on hand to meet the nursing needs of the residents. Report needs to the Nurse Supervisor. Order prescribed medications, supplies, and equipment as necessary, and in accordance with established policies. Ensure that narcotic records are accurate for your shift. Notify the Nurse Supervisor of all drug and narcotic discrepancies noted on your shift. Review medication cards for completeness of information, accuracy in the transcription of the physician’s order, and adherence to stop order policies.
