How you make a difference The Licensed Vocational Nurse (LVN) provides assistance and collects data in the delivery of patient care in a correctional setting, utilizing the nursing process of assessment, planning, implementation, and evaluation. As part of this role, the LVN directs and guides patient teaching and activities in accordance with their education, demonstrated competencies, and the Nursing Practice Act (NPA). The LVN works closely with other healthcare professionals to ensure high-quality care and compliance with established policies and procedures. Key Responsibilities Assist in patient assessment and treatment planning in accordance with the Nursing Practice Act and Correctional Health Services policies. Administer medications and implement medical plans as directed by the Health Care Practitioner, utilizing proper techniques and procedures. Respond to emergencies and maintain safety through the implementation of established protocols and precautions. Implement nutrition and therapeutic diets and assist in medical procedures as necessary to meet patient needs. Document nursing encounters and communicate information to healthcare staff and supervisory personnel. How you make a difference The Nurse Discharge Planner LVN develops and coordinates clinically informed reentry care plans for CalAIM-eligible individuals to address medical, behavioral health, and substance use needs prior to release. The role conducts basic nursing assessments, reviews health records, coordinates referrals, and facilitates warm handoffs to managed care plans and community providers. The position ensures timely service delivery by documenting services in the EHR, tracking release timelines, activating Justice-Involved aid codes, and collaborating with in-reach partners and Medi-Cal eligibility staff. Key Responsibilities Provide discharge planning services for medical, mental health, MAT, and related programs in accordance with contract requirements and policies. Identify release-planning needs, coordinate medications and medical needs upon release, and arrange referrals and appointments with community providers. Facilitate psychoeducational sessions, benefit applications, and resumption of services to support continuity of care. Maintain working relationships with facility staff, parole, mental health, and medical partners while adhering to security regulations. Document services in the health record, activate Justice-Involved aid codes, and ensure compliance with DHCS CalAIM requirements. Qualifications & Requirements Education Graduation from an accredited School of Nursing is required. Bachelor's degree in Social Work, Business Administration, or other health-related fields from an accredited college or university is preferred Experience Previous experience in developing successful transition plans involving community resources for persons with medical, substance use, and/or mental illness is preferred. Licenses/Certifications Have and maintain current licensure as a Licensed Practical/Licensed Vocational Nurse within the state of employment. Must maintain CPR certification, and any other certifications (such as First Aid) required by contract. Must be able to attain and maintain security clearance to the facility. This role supports patient care activities, provides patient education, and implements treatment plans under the direction of healthcare practitioners. The position administers medications, performs clinical procedures, and monitors patient care in accordance with established protocols and nursing practice standards. The LVN guides care activities consistent with demonstrated competencies and the Nursing Practice Act. Key Responsibilities Assist with patient assessments, care planning, and implementation of individualized treatment plans under the direction of healthcare practitioners. Administer medications, perform laboratory testing and drug screens, and obtain diagnostic tests in accordance with orders and protocols. Maintain controlled substance counts and complete required medication administration and inventory documentation. Respond to health emergencies, apply safety procedures, and assist with medical and minor surgical procedures as needed. Document nursing encounters and communicate patient information with healthcare providers and care team members. Additional Details Candidates should have experience with medication administration and dosing, preferably in a clinic, substance use treatment, or correctional healthcare setting. They should be comfortable administering and documenting controlled medications, completing medication and sharps counts, collecting laboratory specimens and drug screens, monitoring patients for medication reactions or overdose, and promptly reporting clinical concerns to an RN or provider. Experience implementing treatment plans, providing patient education, responding to medical emergencies, and maintaining accurate nursing documentation is also important. Familiarity with MAT medications, withdrawal symptoms, security procedures, and working independently within the LVN scope of practice is strongly preferred.
