Nurse Practitioner/Physician Assistant, Cancer Care Access Program

The Nurs Practitioner(NP)/ Physician Assistant (PA) is part of Dana-Farber Cancer Institute’s Cancer Care Access Program (CCAP), which aims to place Dana-Farber at the forefront of efforts to reduce differences in cancer outcomes across local communities. The NP/PA will provide care to patients in CCAP’s onsite Cancer Diagnostic Clinics located within Federally Qualified Health Centers (FQHCs) throughout the Greater Boston area. The NP/PA will evaluate adult patients referred by primary care providers at participating FQHCs for new, existing, or prior cancer diagnoses; benign hematologic conditions; breast concerns; and other concerning cancer-related symptoms. Clinical responsibilities include cancer screening, diagnostic workups, follow-up care, reconnection to cancer care, referrals for second opinions, cancer survivorship care, and patient education. This role requires travel to local FQHCs to provide onsite clinic services. Bilingual or multilingual proficiency preferred. Prior experience practicing in community-based clinics is a plus. The Inpatient Pain FLEX Nurse Practitioner provides expert, evidence-based care for infants, children, adolescents, and young adults experiencing acute, chronic, and complex pain conditions throughout the hospital. The NP is an integral member of a multidisciplinary team that partners closely with anesthesiologists, surgeons, intensivists, hospital medicine providers, nurses, pharmacists, rehabilitation specialists, psychologists, and palliative care providers to optimize pain management, functional recovery, and patient outcomes. In this FLEX role, the NP will be providing coverage for existing LOAs in various areas of the Pain Team. Responsibilities include: Perform comprehensive assessments, obtain medical histories, conduct physical examinations, and develop individualized pain management plans for hospitalized patients with acute, chronic, postoperative, and medically complex pain conditions. Manage patients receiving multimodal analgesia, including opioid and non-opioid therapies, ketamine infusions, patient-controlled analgesia (PCA), epidural analgesia, peripheral nerve catheters, methadone, and other advanced pain management modalities. Assess and manage patients requiring opioid or sedative weaning following prolonged critical illness or hospitalization. Monitor treatment response, adjust pain management plans, and coordinate ongoing care throughout hospitalization. Job Summary This position is Nights Only, 6p-7:30a, weekend/holiday rotation Under general supervision and direction of the Cardiac Surgeons and Heart Center Intensivists, and in accordance with the rules and regulations of Massachusetts governing Nurse Practitioner or Physician Assistant practice, the Advanced Practice Provider (APP) will be responsible for the care of patients on the Cardiac Surgical Service, primarily those patients that are admitted to the Cardiac Surgery Step Down Unit. • The APP will participate in daily rounds with nurses, other Advanced Practice staff, residents, and multidisciplinary caregivers; perform bedside diagnostic and therapeutic procedures, participate in the development and implementation of care plans, coordinate tests and consultations, implement treatment plans, communicate with referring physicians, perform patient education, and prepare patients for discharge, all while keeping Surgeons and Intensivists updated as required. • The APP maintains open lines of communication between Cardiac Surgeons, Residents, Fellows, PAs, Nursing Staff, Cardiologists, Heart Failure team, Intensivists, and patients/families. Promotes a collaborative relationship and effort between the medical staff and professional nurses toward continuity of patient care and efficient use of resources.
