Job Description Join the University of Chicago Medicine as an Advanced Practice Provider (Nurse Practitioner or Physician Assistant) for our Oncology Rapid Assessment Clinic (ORAC). The Advanced Practice Provider will be responsible for comprehensive direct patient care management and will manage patients by exercising autonomous decision making in the assessment, diagnosis, and initiation of delegated medical treatments. The APP will provide healthcare and patient services that may include educational, research, and clinical support, as well as administrative activities under the direction and supervision of the Cancer Service Line leadership. The APP has acquired the knowledge, skills, and judgment necessary to deliver high quality care to hematology/oncology patients. The Oncology Rapid Assessment Clinic (ORAC) i s an APP-staffed clinic area that cares for cancer patients who require urgent, same day/next day assessment and treatment. Hematology Oncology often require acute care for symptoms of malignancy as well as for the toxicities and side effects of cancer treatments. After an orientation period, the APP will work autonomously and collaboratively with the rest of the ORAC APP team. The APP will utilize evidence based practice in close collaboration with University of Chicago heme/onc faculty, APPs, pharmacists and nurses. This candidate will be trained in the management of both hematologic and solid tumor malignancies, as well as oncologic emergencies (eg neutropenic fever, tumor lysis syndrome, cytokine release syndrome, neurotoxicity due to therapeutic agents, DIC, HLH, sepsis, and cord compression). Your primary focus will be delivering world class virtual first cardiology care to our members in the Devoted Medical Heart Center. In this role you will be working in a next generation virtual heart clinic that dramatically expands access to care for America's most vulnerable seniors. The clinic focuses on providing highly accessible, high quality care for members with general cardiology conditions. As part of the clinic you will also provide dedicated clinical services during part of your clinical time to our heart failure clinic which is a highly tailored, specialty center focused on providing best in class heart failure management including optimizing guideline directed medical therapy, providing health coaching, and managing volume overload. You will be a key member of our interprofessional team. On a day-to-day basis you will work closely with heart failure RNs who are assigned as clinical guides for each patient in the heart failure clinic. You will also work closely with an interdisciplinary team including physicians, pharmacists, social workers and medical assistants. You will utilize and help improve our home-grown technology and electronic health information platform to carry out virtual visits. As an In-House Nurse Practitioner , you will be the primary provider of internal medicine services within a Skilled Nursing Facility (SNF). Master's Degree in Nursing from an accredited program. Strong clinical background in primary care and preventive medicine. Geriatric or adult medicine experience preferred, with a focus on skilled nursing, long-term care, hospice, palliative care, or similar settings. Ability to engage and communicate effectively with elderly and chronically ill patients and their families. Ability to meet a minimum of 12 patient visits per day, with bonuses for surpassing this target. Knowledge of Medicare, Medicaid, and Health Plan benefit structures is a plus. Key Responsibilities: As the In-House Nurse Practitioner , you will provide essential care and leadership within the facility, including: Conducting new admission evaluations and monthly stability visits for long-term residents Performing comprehensive physical examinations and evaluating changes in patient conditions Prescribing medications and treatments as needed Developing patient care policies and facilitating referrals to specialists Coordinating advanced care planning and discharge visits Ensuring timely communication and documentation of patient assessments, treatment plans, and follow-ups Supporting the physician-patient relationship and addressing gaps in care Our NPs work closely with the interdisciplinary team, taking a leadership role in patient care, collaborating with physicians, and ensuring high-quality service delivery in our facilities.
