Director of Telehealth Department Clinical Operations — Telehealth Reports To VP of Clinical Operations / Executive Leadership Location Multi-state (Kentucky, West Virginia, Ohio) — remote/hybrid with periodic on-site facility visits Employment Type Full-Time, Exempt Compensation $120,000–$150,000 base salary (planning range; final band subject to internal leveling and market validation) Position Summary CareMed Group is restructuring its telehealth program into a dedicated, rotating clinical coverage model, and launching Chronic Care Management (CCM) and Behavioral Health Integration (BHI) as new care programs. The Director of Telehealth is the single accountable owner of this program: responsible for building and managing the clinical team, redesigning the platform and workflows that support it, launching and overseeing CCM/BHI operations, and ensuring the program meets its coverage, quality, financial, and retention goals. This is a hands-on operational leadership role. The Director will directly supervise a rotating team of Nurse Practitioners/Physician Assistants (FTE and PRN) delivering after-hours and weekend telehealth coverage, chronic care management, and behavioral health integration services across CareMed's partnered skilled nursing, assisted living, and long-term care facilities. Key Responsibilities Program Build & Operations • Own end-to-end build-out and ongoing management of the telehealth program, including the clinical rotation schedule (weekday overnight and full weekend coverage). • Assess and restructure the current telehealth platform and clinical workflows to resolve existing fragmentation and inconsistent coverage. • Directly supervise the clinician team: scheduling, coaching, performance management, and clinical escalation support. • Serve as the primary liaison between the telehealth team, facility partners, and executive leadership on program performance. CCM & BHI Program Ownership • Own the Chronic Care Management (CCM) and Behavioral Health Integration (BHI) program build: patient eligibility screening, enrollment workflows, and patient consent. • Ensure documentation standards and billing accuracy for CCM (CPT 99490, 99439) and BHI (CPT 99484), including correct application of Medicare Part A stay exclusions and place-of-service rules. • Oversee accurate coding and documentation for after-hours telehealth encounters billed under Subsequent Nursing Facility Care codes (CPT 99307–99310). • Monitor CCM/BHI enrollment volume and encounter volume against revenue targets, and adjust outreach or staffing to close gaps. Quality, Compliance & Financial Accountability • Hold the team accountable to chart closure compliance (95%+ within 48 hours) and clinical audit/quality standards. • Track and report on avoidable hospitalization/ED reduction and 30-day readmission rates for the attributed patient population. • Own the program's financial performance: CCM/BHI and encounter revenue versus target, and the program's overall cost-to-revenue ratio. • Ensure all documentation and billing practices meet CMS and payer compliance standards; serve as the point of escalation for coding/compliance questions specific to telehealth, CCM, and BHI. Team Leadership & Retention • Own the retention strategy for the telehealth team: onboarding, workload distribution, escalation support, and culture. • Monitor and manage PRN utilization to ensure sustainable coverage without over-reliance on the backup pool. • Track team turnover and time-to-fill metrics, and proactively address drivers of attrition. Required Qualifications • Active, unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license in at least one of CareMed's operating states (OK, KY, WV, OH); multi-state licensure or eligibility strongly preferred. • Minimum 5 years of clinical experience in primary care, geriatrics, long-term care, or a related acute/post- acute setting. • Minimum 2–3 years in a clinical leadership, supervisory, or program management role overseeing other clinicians. • Direct experience with telehealth service delivery, including platform-based virtual care in skilled nursing, assisted living, or similar settings. • Working knowledge of CMS billing and documentation requirements for Chronic Care Management, Behavioral Health Integration, and Transitional Care Management. • Demonstrated ability to manage a rotating, multi-shift clinical schedule (including nights and weekends) across multiple states or regions. Preferred Qualifications • Prior experience launching or scaling a CCM or BHI program from the ground up. • Familiarity with Subsequent Nursing Facility Care coding (CPT 99307–99310) and place-of-service billing rules for facility-based populations. • Experience with EMR/telehealth platform selection, configuration, or workflow redesign. • Background in value-based care, quality metrics (QAPI, MDS, star ratings), or population health management. • Experience managing PRN/float staffing pools and rotating shift schedules in a healthcare setting. Knowledge & Skills • Clinical documentation and coding: working fluency in CCM (99490/99439), BHI (99484), and telehealth E/M coding (99307–99310), including eligibility rules (e.g., Medicare Part A stay exclusions). • Staffing & scheduling: ability to design and manage sustainable rotating coverage models, balancing FTE and PRN capacity against coverage demand. • Quality & compliance: comfort operating against KPI targets spanning coverage, quality, financial performance, and team health, not productivity volume alone. • Cross-functional collaboration: ability to work with facility administrators, DONs, billing/compliance teams, and executive leadership simultaneously. • People leadership: coaching, performance management, and a demonstrated commitment to team retention and culture. • Data fluency: comfortable reviewing enrollment, revenue, and quality dashboards, and translating trends into operational decisions. Key Performance Indicators The Director of Telehealth will be evaluated against a defined KPI set spanning four categories, consistent with CareMed's broader retention and value-based care strategy: • Coverage & Operations: shift coverage rate (target 100%); time-to-assessment for acute alerts (within facility-defined SLA). • Quality / Value-Based Care: CCM+BHI enrollment rate; chart closure compliance (≥95%); reduction in avoidable hospitalizations/ED visits; 30-day readmission rate. • Financial: CCM/BHI and encounter revenue versus target; program cost-to-revenue ratio. • Team & Retention: team turnover rate (target below the ~18.5% national hospital average); time-to-fill open roles; PRN utilization rate. Work Environment & Schedule This role is primarily remote/telehealth-based, with periodic on-site visits to partnered facilities across CareMed's multi-state footprint. While the Director is not scheduled into the fixed clinical rotation, availability for occasional escalation support outside standard business hours should be expected given the nature of after-hours coverage oversight.

WOUND NP ROLE AND RESPONSIBILITIES: Provide wound care services for long-term care patients residing in skilled nursing facilities. Prescribe appropriate medications, including controlled substances in accordance with KY Law. Order, review, and/or follow-up with treatments and diagnostic testing. Communicate with the nursing home staff on a regular and as needed to discuss changes in condition. and/or the care plan. Collaborate with the primary care physician. WOUND NP QUALIFICATIONS AND EDUCATION REQUIREMENTS Current, unrestricted NP license in the state of Kentucky. Board Certification. DEA licensure (Optional) Clinical background in adult, family, or geriatric specialties. Clinical background of wound care; including but not limited to chronic, debilitating, stalled, or infected wounds Must be comfortable with diagnosing and treating diabetic, arterial, venous, and mixed etiology wounds or Kennedy Ulcers. Must be comfortable staging pressure injuries according to the NPIAP Must be familiar with wound care best practice and advanced wound care products; collagens, alginates, foams, silvers, etc. Must have experience with wound vacs and other DME, such as pressure relieving/ reducing devices Strong preference to having wound certification or be open to getting certified Experience working in a long-term care setting. Ability to work with an EMR. Ability to work within an interdisciplinary care team to coordinate patient care.
