In this role, you will perform in-home wellness and risk adjustment assessments for diverse patient populations with varying healthcare needs. Your work will ensure accurate, comprehensive documentation and support high-quality, value-based clinical care across the state. Communicate findings used to generate post visit summaries for the primary care provider. Identify and help close quality care gaps (e.g., screenings, labs, follow-ups). Collaborate enthusiastically with the broader care team to enhance clinical quality and patient experience. Travel Requirements Full-time providers support the entire state of Oregon. Daily travel expectations remain within a 55-mile radius of the assigned home location. Statewide travel will be scheduled with a minimum of two weeks’ notice. Compliance & Professional Standards Adhere to HIPAA regulations and safeguard protected health information (PHI). Maintain accurate, timely, and compliant EMR documentation. Perform additional duties as assigned in alignment with organizational goals. Qualifications Active, unencumbered Nurse Practitioner (NP) license in Oregon. Minimum 3 years of direct patient care experience (primary care, adult/geriatric preferred). EMR proficiency is desired. Prior in-home risk assessment experience is a plus but not required. Bilingual proficiency is an advantage. The ARNP/PA-C is a certified physician assistant or advanced registered nurse practitioner who provides evaluation and medical management to Heme/Heme Malignancy patients at Fred Hutch Cancer Center. They follow patients during their chemotherapy and provide medical supportive care, as well as surveillance after completion of treatment. The ARNP/PA-C assesses and manages basic health needs of patients, addresses treatment toxicities, performs procedures, monitors for cancer progression or recurrence, and consults with professional peers as needed. They work collaboratively with the treatment team to coordinate care of their patients. All clinical responsibilities are performed within the scope of practice, institutional privileges and under the supervision of an attending physician. We are currently seeking a full scope Certified Nurse Midwife to join our midwifery practice. This position is a full-time 1.0 FTE position with a 12 month (July 1 - June 30) academic position on a yearly appointment with an anticipated start date of January 1, 2027. Teaching Associates hold annual appointments that align with a 12-month service period (July 1-June 30) and may be reappointed to subsequent annual terms. Faculty with 12-month service periods are paid for 11 months of service over a 12-month period (July-June), meaning the equivalent of one month is available for paid time off. The base salary range for this position will be: $10,834 - $14,667 per month ($130,008 - $170,004 annually) with competitive total compensation commensurate with experience and qualifications, or as mandated by a U.S. Department of Labor prevailing wage determination. Certified Nurse-Midwife (CNM) Position Responsibilities – High Acuity Midwifery Care The successful candidate will be expected to provide full-scope midwifery care within an all-CNM practice group, including independent management of women's health with a primary focus on pregnancy, birth, and postpartum care, as well as family planning, gynecologic, and primary care of women. This individual will deliver high acuity intrapartum and obstetric care, including co-management of medically and obstetrically complex patients (e.g., preeclampsia and hypertensive disorders, diabetes in pregnancy and other high-risk conditions) in collaboration with Maternal-Fetal Medicine (MFM) faculty and other providers. Candidates will be expected to function effectively within a collaborative, team-based model that supports rapid escalation of care when clinical acuity increases; manage hospital labor assessment, obstetric triage, and admission; oversee normal and abnormal labor, including advanced fetal heart rate monitoring and interpretation; and perform cervical ripening and labor induction. The successful candidate will perform vaginal birth and laceration repair (first-, second-, and as appropriate to training); limited first-, second-, and third-trimester obstetric ultrasound as appropriate to training and experience; cesarean section first surgical assist; and IUD and subdermal contraceptive implant insertion and removal. This individual will also provide postpartum care and discharge of mothers, including management of the higher-acuity postpartum patient. Performance Expectations: Performs chart reviews for patients determined to be at increased risk for perioperative complications from medical history or abnormal testing. This clinic provides a broad range of services including GI, Lung, GU, Breast, and Hematologic Malignancies and Radiation therapy.
