The primary purpose of the Advanced Practice Provider (APRN/PA), in the Department of Hospital Medicine is to promote and maintain the health of HM patients across the care continuum throughout the institution supporting the inpatient hospitalist programs. As the Department of Hospital Medicine is a service that operates 24/7/365, nocturnal shifts will also be supported by the hospitalist advanced practice providers for after-hours and weekend coverage, rotating holidays and COVID coverage. Obtains a comprehensive health, developmental, and family history on patients as documented in the patient’s medical record. Assess the patient’s physical status through the physical examination as documented in the patient’s medical record. Differentiates between normal and abnormal findings as documented in the patient’s medical record. Performs or recommends age-appropriate health screening procedures as documented in the patient’s medical record. Provides health maintenance and health promotion for women of all ages as documented in the patient’s medical record. Performs and evaluates diagnostic procedures and laboratory tests as documented in the patient’s medical record. Analyzes multiple sources of data and formulates a differential diagnosis in collaboration with physicians as demonstrated in the written documentation. Follows established delegation protocols to manage specific patient problems. Formulates a plan of care in collaboration with other health care professionals and documents the plan in the patient’s medical record. Accurately evaluates patient’s responses to interventions and modifies the plan of care demonstrated by written documentation. Consistently demonstrates effective communication skills and works to eliminate communication barriers. Consistently serves as an educational resource for the patient and family, community, and other health care professionals. Consistently applies new knowledge relevant to clinical practice. Demonstrates current professional, managerial, and/or technical knowledge and skills. Demonstrates NP role behaviors consistent with the development of advanced nursing practice. Participates in departmental Quality Improvement program annually. Participates in at least one QI aspect of care, which shows improvement, related to patient outcome.
