Summary The Department of Anesthesiology, Section of Critical Care Medicine is seeking a qualified candidate for Advanced Practice Clinician position in a critical care clinical role. This Advanced Practice Clinician will provide medical care to patients with complex acute, critical, and/or chronic health conditions in a hospital setting in one or more specialized areas under the direction of a physician. Candidate will be required to provide clinical excellence, scholarly enthusiasm, and a track record of excellent clinical performance. The provider will have direct patient care responsibilities. The position will report to a BCM department head and the SVP and Dean of Clinical Affairs. Key Functions: Be able to conduct comprehensive assessment of patients throughout the continuum in the inpatient setting based on age and diagnosis, including psychological status. Perform and document history and physicals, order diagnostic studies, therapeutic procedures, prescribe medications, and participate in collaboration and direct reporting of patient information to the medical staff. Proficient use of Epic in the delivery of care. Working Conditions: This position requires working in Critical Care Units. Job Purpose Provides advanced nursing care to patients in a clinical setting in an area of specialization under the direction of a physician. Job Duties Obtain detailed history and perform physical examinations. Perform, order, and interpret diagnostic procedures in accordance with relevant health concerns of the patient. Prescribes appropriate pharmacological and non-pharmacological treatments. Maintain records and write orders and progress notes in patient charts as to status, treatments, and procedures. Monitors and ensures accuracy of recording on patient medical records. Provide follow-up health maintenance care to patients in accordance with protocols established with the supervising physician. Communicate with physicians, patients, and the family members of patients concerning patient care. Collaborates with providers, and practice staff in identifying appropriate patients for care management. Perform other job related duties as assigned. He/she demonstrates a high degree of clinical expertise in working with patients with acute and chronic illnesses or acute injuries commonly encountered in both the pediatric and adult patient population. He/she is responsible for assessment, diagnosis, treatment, management, education, health promotion and care coordination for patients and families with acute and chronic health needs. Essential Functions and Responsibilities: Provide care generally on an outpatient basis and such pre-operative care in minor surgery normally performed on an outpatient basis Responsible for the delivery of care, treatment, and referrals. Arrange for referral resources for those medical specialties not provided by the organization for patients of the clinic Evaluate patients, based on medical training and/or specialty, to determine the medical condition of the patient, through the performance of physical examinations, thereby, establishing a diagnosis and proposed treatment plan Perform such clinical procedures for patient's benefit which are within scope and skill of practitioner and which are economically feasible Communicate information to patients regarding diagnosis and treatment goals Maintains accurate, complete, concise and timely documentation in the electronic medical record Perform other job-related duties, as may be assigned, by the Medical Director Requirements and Qualifications: Current, unrestricted PA or NP license in the state or the ability to obtain Ability to obtain DEA licensure Knowledge of treatment and drug interactions Excellent communication and interpersonal skills Community Care Partners is an Equal Opportunity Employer (EEO). #APP
