Facilitates a positive and caring relationship with others by utilizing trust and mutual respect. Establishes a therapeutic relationship between patient, family and self, based on empathy and trust. Promotes a positive patient experience. Seeks help from appropriate resources as needed. Maintains current knowledge of and practices within the scope of hospital policies. Clarifies any unclear communication between members of the healthcare team. Uses proper channels of authority in communicating information. Collects Data related to physical, psychological and psychosocial health status. Develops a plan of care and teaching plan based on assessed needs of patient and family. Revises same as diagnosis and needs dictate. As part of the nursing process, performs admission, handoff, transfer and discharge procedures. Coordinates with physician’s plan of care, the admission assessment and discharge needs. Maintains compliance in RQI. Assumes all other duties and responsibilities as necessary. Charge Nurse: In addition to above essential functions, the charge nurse will also : Complete daily nurse leader rounds. Perform shift checks as assigned. Assists with completion of chart audits for nursing documentation as required. Complete staff assignments taking into account the skill level and acuity of patient assignment. Manages throughput on the nursing unit and make changes to assignments as needed. Reports to the nursing unit manager any problems in regards to performance, safety, staffing, behavior, etc. Assists with conflict resolution in the absence of the nursing manager in collaboration with the house supervisor. Role models positive behaviors with staff including integrity, fairness and consistency. Agent for change for the department. Assigned and assist as super user for completion of incident report. Minimum Education/Experience Required: Current licensure in the State of Ohio or if working in WV will need a WV license as a Registered Nurse. BLS required upon hire. ACLS within 1 year of employment required. Special Knowledge, Skills, Training: Verbal, clerical, numerical, and manual dexterity, color vision and near visual acuity, proper lifting techniques and body mechanics. Description A Registered Nurse Practitioner engaged to work on an as-needed basis for patient care delivery. Responsibilities Principal Duties and Responsibilities 1. Clinical Practice a. Develops and implements a collaborative practice agreement in the specific clinical area and performs a periodic review of the collaborative agreement. b. Assesses the health status of patients/families by means of comprehensive health history and physical examination. c. Performs a patient record review with the collaborating physician in a timely fashion but not less than every three months j. Manage a clinical specialty program for a select patient population across the hospital continuum and provide outcome data for program evaluation and performance improvement. 2. The establishment of Collaborative Practice Agreement Chapter 257 of the Laws of 1988 of New York State which took effect April 1, 1989, authorizes Nurse Practitioners certified by the State Education Department to use the title "Nurse Practitioner" [for Nurse Practitioner job description, see appendix] and engage in the expanded practice of nursing, which may include the performance of primary health care services and the issuance of prescriptions. Practice agreements and practice protocols must be established and maintained in the practice settings of the Nurse Practitioner and collaborating physician where they shall be available to the Department for inspection. 2. Practice Protocols Practice protocols shall identify the area of practice to be performed by the Nurse Practitioner in collaboration with the physician and shall reflect accepted standards of nursing and medical practice. Protocols shall include provisions for case management including diagnosis, treatment, and appropriate recordkeeping by the Nurse Practitioner; and may include such other provisions as are determined by the Nurse Practitioner and collaborating physician to be appropriate. These protocols will be determined by area of specialty. 3. Practice Agreements Practice agreements shall include provisions for the following: a. Patient Assignment Patients are assigned to the Nurse Practitioner or physician according to the needs of the patient. b. Diagnostic Tests Laboratory tests may be ordered according to the initial work-up guidelines found in the reference protocol. Collaboration is not necessary for routine diagnostic procedures. However, consultation and documentation should be completed before more extensive tests are ordered, e.g., nuclear medicine tests, MRI, CT scan, IVP, etc. c. Consultation Nurse Practitioners consult with and refer patients to subspecialty clinics as needed. d. Prescriptive Practice Nurse Practitioners prescribe medications according to the New York State Laws and Regulations for prescription writing and record-keeping (see Appendix). e. Charting Nurse Practitioners are responsible for documenting findings, consultations and plans of care in the patients' medical records. The role of the Professional Staff Nurse encompasses leadership, partnership, collaboration, and supervision. The Professional Staff Nurse establishes and maintains collaborative relationships with physicians, other health care providers, patients, and their families, to achieve desired patient outcomes throughout the continuum of care. Practices solid communication skills, and is able to articulate and translate the patient's condition to other care providers and to negotiate and make recommendations for changes in patient care and unit practices. Patient documentation is comprehensive and promotes communication between caregivers. Incorporates national professional organization as well as business unit and health system's goals to improve patient safety, quality and satisfaction. Creates a caring and compassionate patient focused experience by building healing relationships with patients, families and colleagues. Minimum 6 months of RN experience required. BSN preferred. Oncology experience preferred. IV Skills preferred. The individual must be able to demonstrate the knowledge and skills necessary to provide care and/or interact appropriately to the ages of the patients served by his/her assigned unit as specified below. They must also demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient’s status and interpret the appropriate information needed to identify each patient. Ability to establish and maintain positive, caring relationships with executives, managers, physicians, non-physician providers, ancillary and support staff, other departments, and patients/families. Ability to work productively and effectively within a complex environment, handle multiple/changing priorities and specialized equipment. Good clinical judgment with critical thinking, analytical and problem-solving abilities required as related to various aspects of patient care. Critical thinking skills necessary to exercise and to lead others in application of the nursing process. Mobility and visual manual dexterity. Physical stamina for frequent walking, standing, lifting, and positioning of patients. Licensure, Certifications, and Clearances: UPMC approved national certification preferred. Current Pennsylvania licensure as a Registered Professional Nurse in PA, OH or NY required. CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire. Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR) Registered Nurse (RN) OR Temporary Practice Permit (TPP) Current licensure either in the state where the facility is located or, if the facility is in a state covered by the multistate Nursing Licensure Compact (NLC) agreement, a multistate license issued by a participating NLC state. Hires and current employees working on an out-of-state NLC license who later change their residency to the state where the facility is also located will have 60 days upon changing their residency to apply for licensure within that state.
