Nurse Practitioner - Cancer Center - Mount Sinai Queens - Full Time Days

Description A nurse practitioner (NP) as described in Title VIII, Article 139 section 6910 of the New York Education Law is a RN who has earned a separate certification as an NP through advanced clinical nursing education in a distinct specialty area of practice. Nurse practitioners are independent practitioners who diagnose, treat, and prescribe for a patients condition that falls within their specialty area of practice. NOTE: Medical orders, progress notes and treatments plans are not required to be countersigned by a physician. PRINCIPLES DUTIES AND RESPONSIBILITIES CLINICAL PRACTICE EDUCATION REQUIREMENTS Licensure: New York State License with current registration as a registered nurse. State Certification: Certified by the New York State Department of Education as a Nurse Practitioner with current registration as follows: a.Is a graduate of New York State registered Nurse Practitioner Program or equivalent OR is certified as a Nurse Practitioner by a national association via examination. b.Has completed a pharmacy component of not less than the required semester hours or the equivalent AND has been instructed in New York State and Federal Laws relating to prescriptions and recordkeeping (3-hour course). c.Has a Collaborative Practice Agreement with a physician and designated protocols if less than 3600 hours experience as a practicing Nurse Practitioner. OR Has a Collaborative Relationship with a physician based on employment in clinical practice in the NP role at Mount Sinai Hospital. If there is a documented work history as a Nurse Practitioner with greater than 3600 practice hours. Please note all NPs regardless of their status will be placed on an FPPE initially) Education: Master's degree program designed to prepare a NP to diagnose, treat, and prescribe for a patients condition that fall within their specialty area of practice or the successful completion of an approved supplemental education program, i.e., post-Masters NP certificate. Experience: Previous progressive experience relevant to the area of clinical practice required. For all NPs hired effective June 1, 2019, National Board Certification in Advanced Practice Nursing required. For NPs who bill for professional services, National Board Certification is required regardless of hire date. The Board Certification requirement may be deferred up to 1 year if approved by Chief Nursing Officer or designee. Credentialed through the Credentialing Committee of the Medical Board with approved Delineation of Privileges Drug Enforcement Administration (DEA) certificate and Basic Life Support (BLS) certification required Advanced Cardiac Life Support (ACLS), Pediatric Advanced Life Support (PALS), Neonatal Advanced Life Support (NALS), The Neonatal Resuscitation Program (NRP) certificates as required by specialty. Experience Requirements At least two years of nursing experience in area of specialty practice applied for. Nurse Practitioner experience of at least one year preferred. Responsibilities 1. Assesses the health status of patients/families by means of comprehensive health history and physical examination. 2. Initiates and orders selected therapeutic and diagnostic procedures, consultations and laboratory examination, prescribes medication regimens, interprets findings and alters plan of care according to accepted standards of care and practice guidelines 3. Educates and counsels in the areas of health promotion, maintenance, disease prevention, and management of acute/chronic illnesses. 4. Documents findings and plan of care in the patient's medical record. 5. Consults with physician according to established standards, policies, procedures, and protocols. 6. Demonstrates the knowledge and skills necessary to provide care, based on physical, psychosocial, educational, safety, and related criteria, appropriate to the age of the patients served in assigned area. 7. Initiates referrals to specialty services and fosters continuity of care. 8. Participates in team meetings and conferences to enhance an interdisciplinary approach to care delivery. 9. Maintains patient/employee confidentiality in the management of information. 10. Develops the plan of care based on the diagnoses and individual patient needs: records, reports, and interprets the clients responses to the plan of care. 11. Performs a patient record review with the collaborating physician in a timely fashion but not less than every three months if utilizing a collaborative practice agreement. 12. Completes the Focused Professional Practice Proctoring Evaluation (FPPE) or the Ongoing Professional Practice Proctoring Evaluation (OPPE) as required by Medical Staff for continued credentialing status. 13. Practices as a member of the allied health staff according to the rules and regulations of the Medical Staff and the bylaws of the Hospital Staff. Formulates an evidence-based treatment plan according to the interpretation and evaluation of all patient data. Collaborates with other healthcare team members involved in patient treatment to ensure continuity of care. Initiates arrangements for hospital admissions and discharges patients as indicated by clinical status. Manages the clinical course of patients with cancer with acute or chronic complaints and end-of-life and other health concerns. Performs core clinical practice competencies specific to the patient population, and promotes, directs, evaluates, and manages safe patient care for patients being treated in the chemotherapy infusion area, and provides patient teaching and coordinates counseling. Prescribes medications and administers orders as indicated by patients’ clinical status. Serves as a cancer pain management resource to the medical center in both outpatient and inpatient settings. Recognizes and initiates lifesaving procedures for oncologic emergencies and brings emergencies to the immediate attention of a physician. Participates in the tumor board and clinical research studies as determined by the needs of the program and participates in staff development and in-service education as requested. Promotes and participates in the delivery of patient and family education on the cancer diagnosis, treatment, expected outcomes, and end-of-life issues such as do-not-resuscitate orders, healthcare proxies, and advance directives; and assists in the completion of patients’ disability and insurance forms as needed. Ensure proper PPE is always worn while on duty including but not limited to, face mask, gloves, gown, isolation gown, NIOSH-approved N95 filtering facepiece respirator or higher, if available), and eye or face shield. Complete all donning and doffing tasks in a safe acceptable method and discard of used PPE accordingly. Responsibilities Assist with patient care under the direction of physicians. Prepare patients for office visits and procedures. Preforms a variety of patient care activities, such as vaccine/medication administration, phlebotomy, and point of care testing. Provides indirect patient care via phone or EHR by addressing patient questions, processing medication refills and scheduling follow-up appointments. Utilize electronic health record for efficient care. Qualifications One year experience as a clinic back office medical assistant preferred. Education: Completion of a medical assistant program or equivalent experience; or Completion of a formal medical services training program of the United States Armed Forces; or Completion of Basic or Advanced Oregon Emergency Medical Technician program (Oregon only) Licensure MA certification required within one year, from either the American Association of Medical Assistants (AAMA), the American Medical Technologists (AMT), the National Center for Competency Testing (NCCT), or the National Healthcare Association (NHA). MA certification requirement may be met instead by current Oregon Emergency Medical Technician (EMT) license and national EMT registration (NREMT). Oregon only. This role is ideal for an experienced oncology nurse who is passionate about providing exceptional care to pediatric patients and their families. The RN will work collaboratively with a multidisciplinary team to deliver high-quality, family-centered care in the outpatient setting of the Children’s Cancer & Blood Disorders and Peds Day Treatment Unit. These units operate Monday-Friday, day shift only with flexibility based on patient needs. Ideal candidate has a minimum of two years of nursing experience and one-year recent pediatric oncology experience and the availability to work a minimum of 2 shifts per week. Qualifications Education: As required by licensure. BSN or degree in progress strongly preferred. BSN may be required for specific assignments; experienced RNs without a BSN may be hired based on clinical experience with approval of the site CNO. Experience: One-year registered nursing experience or completion of either the Legacy Health RN Residency program or a nursing program that is equivalent in content, curriculum and duration. Additional clinical experience requirements may vary dependent upon the specific department in which the position is utilized. Knowledge/Skills: Demonstrated ability of positive interpersonal and communication skills with colleagues, patients, families, physicians and the community.
