Performs basic office laboratory procedures such as urinalysis, urine culture, hematocrit, sed rate, mono spot, pregnancy tests, throat cultures and related procedures. Performs basic clinical procedures such as venipuncture/phlebotomy. Prepares documents and administers vaccines/non-intravenous injections as directed. Performs a variety of clerical functions to support office such as typing, filing, scheduling appointments, making phone calls and related duties as required or directed. Coordinates patient flow and prepares patient for examination. Takes and records vital signs and EKG’s. Documents intake assessment. Assists physician and others in clinical procedures as required or directed. Assist in triaging medical calls as per office protocol. Registers patients for each patient visit, utilizing computer based registration/scheduling system. Verifies patients insurance coverage and eligibility. Maintains assigned office and/or clinical supplies including vaccines. Assures accurate inventory of supplies based on approved levels. Rotates stock as required. Initiates requisitions for re-supply or special items. Issues and verifies patients managed care referral. Collects copays from patients for visits, maintains records and reconciles daily deposits. This position does require travel through the service area: Henry, Des Moines, Lee, Louisa, and Washington. Job Description · Work directly with program participants to assess physical health care needs and coordinate with members’ health care professionals on those needs. · Ensure care given to those with complex mental health needs is appropriate and coordinated, incorporating best practice for treatment, risk assessment and multi-agency service coordination. · Act as the team expert on the medical issues of the members · Need to use patient registries, population management, and gaps in care analysis tools in serving members. · Communicate and coordinate with primary care and specialty care providers to review and plan care for members. · Assist in the coordination of medically related service needs and supports. · Understands self-management tools, usage, and need, and can develop/organize for the population being served · Follow and enforce standards of ACT programming · Enter data as required for member outcomes, comfortable with computer use. · Implementing preventive, as well as remedial, health and wellness plans. · Cross-training of other team members in the use of health and wellness interventions as individually indicated. · Work closely with other members of the team and staff from other key agencies. · Provide education and training to agency staff regarding health issues. · Cooperates and collaborates with program area staff, volunteers, and other Agency staff. · Abides by all specific program and Agency procedures, policies, and requirements. · Develops personal and program-related skills through participation in internal and external training opportunities, including printed material and audio and/or visual media. · Supervise the provision of medication monitoring/ administration by staff, including supervision and providing training · Complete nursing assessments and medication reconciliation for clients · Provide medical and psychiatric education to clients and their families · Conducts medical/health/wellness assessments to identify health care needs and assist in developing health care goals with the treatment team. · Track required healthcare assessment and screenings · Knowing physical health aspects of client care, including weight, nutrition, medication education, assessing and education of side effects, risks and benefits of medication · Providing clients with community resources Benefits · Competitive salary · Medical, Dental, and Vision plans · Paid Time Off · 401k plan Qualifications · RN or LPN current Iowa license · 2 years of nursing practice with those with complex mental health needs · Knowledge on psychotropic medications and DMS-IV diagnoses · Valid driver’s license Elevate is an equal opportunity/affirmative action employer.
