Overview The Clinic Nurse I is accountable for interpreting the plan of Medical care, assessment of patients, devising the patient care plan, providing individualized nursing care, and evaluating nursing care for groups of patients under the direction of the nurse in charge. Plans for continuity of patient care considering all available resources and documents actions taken. Refers patients to social worker, and agencies as required. Provides a safe and secure environment for the patient. Implements the patient care plan by formulating objectives for ambulatory care, assigning tasks to clinic personnel, and continuously assessing and evaluating patient care and patient responses. Performs treatments and procedures and patient care which may require expert skill and judgment in accordance with established standards. May attend physicians during exams, communicates pertinent information about patient to the physician, and assists the physician with treatments and procedures. Performs certain special procedures for which specialized training and supervised experience have been received. Provides effective patient and family education. Documents all nursing intervention. Prepares and maintains reports, charts, records, and forms as required by Ambulatory Clinic, state, and federal regulations, JCAHO, and CLIA. May accompany patients who are transferred to another medical facility, or the Emergency Room. Notifies doctor, and/or family when patient’s condition changes. Reviews work performed by LPNs and Clinic Clerks to ascertain that treatments and procedures are being performed in compliance with the patient care plan and established policy and procedure. Participates in teaching activities for personnel and nursing students. Participates in the orientation of new employees. Participates in committee meetings as requested. Attends inservice programs. Assumes responsibility for personal professional development. Assists in the development and revision of policies and procedures. Respects the dignity and confidentiality of all matters relating to the patients. Performs other related duties as required within the scope of the job classification. Qualifications Education • Graduate of an accredited school of practical nursing. Overview The Department of Internal Medicine at Macon & Joan Brock Virginia Health Sciences at Old Dominion University is seeking a Licensed Practical Nurse (LPN) to join the team. Provides nursing care for patients and assisting physicians. Clinical and administrative duties include: Triage patients [vital signs, height/weight, medical/social history, medication reconciliation] Stock/prepare exam rooms Retrieve imaging and laboratory results Process work task/message ques in the electronic health records [EPIC], and the patient portal [myChart] Ability to work in our clinical and administrative areas with the Department of Medicine. Responsibilities Perform patient intake including obtaining vital signs, reconciling medications and allergies, documenting chief complaints, administering medications and immunizations according to approved protocols, observing for adverse reactions, assisting providers with examinations, minor procedures, diagnostic testing, and implementing standing orders. Provide basic patient care and document all nursing interventions in the electronic medical record. Manage patient-related tasks and follow-up activities including responding to patient messages and telephone calls, communicating test and laboratory results, answering patient questions, coordinating referrals and specialist appointments, interacting with family members and healthcare providers, documenting all patient communications, notifying providers of critical results, and ensuring same-day response to patient inquiries when appropriate. Review charts for completeness, monitor test results and task completion, reconcile discharge medications, manage prescription refills, obtain medication prior authorizations, administer PPD testing as needed, and ensure accurate documentation and billing activities within the electronic medical record. Provide education regarding medications, treatments, nutrition, and disease management; address barriers to care; support drug assistance programs; coordinate home health services; distribute and track home health orders; enter home health charges for billing; and promote patient understanding of physician instructions and treatment plans. Overview Full time position for an advanced medical assistant, providing outpatient care for our patients and assisting our physicians in all clinical areas of the practice. Responsibilities Train and orient new medical assistants. QA Review of patient charts obtaining all pertinent documentation for return and postpartum patients. Updating patient prenatal lab forms for all follow up patients. Schedule outside patient referrals. Maintain/order supplies for the clinic to include weekly checks for expired materials/medications and/or materials close to expiration. Takes vital signs such as height, weight, temperature, and blood pressure. May draw blood and give injections under the immediate supervision of a Physician or RN. Chaperones when appropriate and assists during certain minor surgeries and diagnostic procedures, such as Pap Smears. Maintains clean, well-equipped examination rooms; cleans and sterilizes instruments. Stocks examination rooms and triage area with medical supplies. Inventories and reorders supplies. Document all information in EMR. Prepare education packets. Check EMR tasks daily. Review patient lab results, discuss abnormal findings with the physician and follow up with patient in regard to treatment as necessary PATIENT CONFIDENTIALITY MUST BE MAINTAINED AT ALL TIMES. OTHER DUTIES AND RESPONSIBILITES Answers incoming telephone calls and refers calls as necessary. Also responsible for taking messages, paging doctors when appropriate, handling hospital consults, and answering inquiries. Schedules appointments with doctors, calls patients to remind and confirm appointments; reschedules patients from the bump list, and schedules residents’ clinics where appropriate. Prepares charts for the next day; creates new patient charts, files and pulls charts and checks charts for referrals and authorization prior to visit. Also prepares charge tickets. Greets patients in a courteous manner, completes clinical intake, and notifies nurses/medical assistants/MDs of arrivals. Checks-in and checks-out patients, obtains and writes referrals, when appropriately trained and directed, and schedules outside appointments with specialists. Gives directions to patients and visitors. Maintains order in reception room. Copies medical records. Provides back-up coverage for medical assistant, billing, etc. Assists RNs and LPNs with administrative duties. Handles pick-up and drop off of medications. Phone triage. Performs other duties as assigned, according to the individual departmental needs. Qualifications Successful completion of a Medical assistant program (or equivalent military training) and certification (CMA or RMA) is required. Must have working knowledge of computers.
