Summary: The Registered Nurse (RN) plays a critical role in executing InterCommunity mission to promote whole person health. Essential Duties & Responsibilities: Provides direct patient care, carrying out orders given by my medical provider, which may include obtaining medical histories, obtaining vital signs, performing office lab testing and phlebotomy/ venipuncture, EKGs, wound care, and which includes but is not limited to medication administration. Conducting nursing visits or educational groups as ordered by the medical provider Maintains accurate, detailed reports and records. Records and reconciles patients' medical information. Monitors, records, and reports symptoms or changes in patients' conditions, assess efficacy of prescribed medications. Ensure gaps in care for screenings, chronic care tests (such as HgbA1c for diabetes), BMI, vaccinations, are completed in timelines established by best practice. Facilitates effective crisis intervention. Perform safety assessments and triage for patients in crisis. Consults and coordinates with healthcare team members to assess, plan, implement, or evaluate patient care plans. Modifies patient treatment plans as indicated by patients' responses and conditions and collaborate within a multidisciplinary team to achieve best patient outcomes Maintains an active 10- patient panel for Complex Care Management (CCM). Prepare patients for electrophysiology-related diagnostic procedures and testing by providing instructions, assisting with gowning and positioning, maintaining privacy, and applying monitoring equipment as directed. Observe patients before, during, and after testing; recognize and immediately report symptoms, abnormal vital signs, rhythm changes, or other safety concerns to the supervising nurse, provider, or appropriate clinical team member. Apply, activate, and remove ambulatory cardiac monitors, including Holter, event, mobile cardiac telemetry, and patch monitors, as ordered and within demonstrated competency. Educate patients and caregivers regarding monitor use, symptom recording, skin care, activity precautions, troubleshooting, device return, and when to contact the clinic. Manage monitor inventory and related workflows, including device assignment, enrollment, return tracking, data routing, and follow-up on overdue or incomplete studies. Support clinic workflow by rooming patients, preparing records and orders, coordinating diagnostic appointments, responding to patient messages, and assisting with referrals or prior authorizations as assigned. Document patient preparation, vital signs, education, device information, procedure support, and other assigned clinical activities accurately and promptly in the electronic health record. ***This position is eligible for a $15,000 SIGN-ON BONUS*** Job Summary: Physician Assistants (PA) provide health care services to patients who may have acute episodic conditions or chronic problems, as well as those seeking health evaluation, preventive care and/or education and counseling. Under physician supervision, PAs provide care in areas within KP in primary care or specialty departments consistent with their educational preparation following the Regional Practice Guidelines and Protocols for Ambulatory and Hospital-Based Practice. They perform complete physical examinations, including ordering, interpreting and evaluating diagnostic tests and examinations. Assume on-going responsibility for each patient from discovery of the disorder to recovery. Diagnose and treat both chronic and episodic disorders including complications of otherwise normal processes. Establish and document the health care plan and diagnosis, maintain records of each case that are sufficiently complete for any physician reviewing them to evaluate the effectiveness of the plan.Consistently demonstrates the knowledge, skills, abilities and behaviors necessary to provide superior and culturally sensitive service to each other, to our members and to customers, contracted providers and vendors.
