Works under the supervision of the Nurse Manager and the Director and is responsible for the assessment, planning, implementing, and evaluating the total nursing care of assigned patients. Partners with patients and their family to develop, implement and evaluate a plan of care, treatments and procedures that contribute to physiological and psychosocial outcomes of the patient. Incorporates evidence based standards of practice and collaborates with interdisciplinary team in care coordination. Facilitates safe patient transition to next level of care as part of plan of care. Demonstrates sense of urgency to facilitate patient throughput to ensure patient receives consistent, competent and ethical care. Focuses on quality outcomes of patient when addressing service needs to ensure evidence based quality care of patient. Demonstrates understanding of diverse patient care populations. Modifies education method to ensure patient/family understanding. Assumes responsibility to coordinate and deliver individualized patient education and discharge specific education to family/caregivers. Evaluates for comprehension and documents. Accurately completes all required clinical and quality documentation consistently per RHS standards and regulatory requirements. Riverside's Nurse Aide Program is intended to transition new hires from Hospitality Assistants to fully licensed Certified Nurse Aides. In this program, you will begin by working as a Hospitality Assistant in a labor pool (as needed) status until your scheduled course begins. The course is an accelerated 4-week program that will teach you the skills and knowledge needed to take and pass your CNA boards. Classes require full-time day shift hours (participants are paid for all course and clinical hours). Following the completion of the program and passing the Virginia Board of Nursing – Nurse Aide exam, you will move into a full-time or part-time CNA position at the location in which you are hired. Full-time CNA positions require a 12-month work commitment and part-time requires an 18-month work commitment upon graduation. Status is determined by facility need. Perform routine charting duties as required and in accordance with our established Charting and Documentation Policies and Procedures. Responsible for completion of admission, discharge, and transfer processes. Prepare and administer medications and treatments if appropriate as ordered by the physician. Review medication record for completeness of information, accuracy in the transcription of the physician’s order, and adherence to stop order policies. Administer professional services such as: catheterization, tube feedings, suction, applying and changing dressings/bandages, packs, colostomy, and drainage bags, care of the dead/dying, etc., as required. Participate in the orientation of new customers/family members to the facility. Arrange for diagnostic and therapeutic services, as ordered by the physician. Notify the customer’s attending physician and family when there is a change in the customer’s conditions. Obtain sputum, urine and other lab tests as ordered.
