Job Summary The RN-Home Infusion Clinical Liaison serves as the clinical liaison between referral sources, the home infusion pharmacy, and the interdisciplinary care team to support timely initiation of home infusion therapy. Performs clinical screening and assessment of referred patients to evaluate appropriateness of home infusion therapy based on diagnosis, prescribed treatment, patient readiness, and home environment considerations. Collaborates with intake coordinators, pharmacists, nursing staff, and providers to coordinate therapy initiation, nursing services, medication delivery, and start of care timing. Educates patients and caregivers on prescribed infusion therapies, vascular access devices, equipment use, and safety considerations to support successful therapy in the home setting. Communicates clinical information and recommendations to referring providers, case managers, and discharge planners to ensure continuity of care and appropriate transitions from inpatient or outpatient settings. Identifies and addresses clinical barriers to home infusion therapy initiation, escalating complex cases or concerns to leadership or medical providers as appropriate. Supports business development efforts by building and maintaining relationships with hospitals, clinics, physicians, and referral partners, serving as a clinical resource to promote home infusion services. Participates in referral source education and outreach activities to increase awareness of home infusion capabilities, clinical appropriateness, and service offerings. Ensures accurate, timely documentation of clinical assessments, patient education, and coordination activities in accordance with regulatory, accreditation, and payer requirements. Participates in quality improvement and performance initiatives related to home infusion clinical services, contributing clinical insight to improve patient outcomes and operational efficiency. Performs other duties as assigned. Supervisory/Management Responsibilities This is a non-management job that will report to a supervisor, manager, director or executive.
