Promotes a positive patient experience. Seeks help from appropriate resources as needed. Maintains current knowledge of and practices within the scope of hospital policies. Clarifies any unclear communication between members of the healthcare team. Uses proper channels of authority in communicating information. Collects Data related to physical, psychological and psychosocial health status. Develops a plan of care and teaching plan based on assessed needs of patient and family. Revises same as diagnosis and needs dictate. As part of the nursing process, performs admission, handoff, transfer and discharge procedures. Coordinates with physician’s plan of care, the admission assessment and discharge needs. Maintains compliance in RQI. Assumes all other duties and responsibilities as necessary. Charge Nurse: In addition to above essential functions, the charge nurse will also : Complete daily nurse leader rounds. Perform shift checks as assigned. Assists with completion of chart audits for nursing documentation as required. Complete staff assignments taking into account the skill level and acuity of patient assignment. Manages throughput on the nursing unit and make changes to assignments as needed. Reports to the nursing unit manager any problems in regards to performance, safety, staffing, behavior, etc. Assists with conflict resolution in the absence of the nursing manager in collaboration with the house supervisor. Role models positive behaviors with staff including integrity, fairness and consistency. Agent for change for the department. Assigned and assist as super user for completion of incident report. Minimum Education/Experience Required: Current licensure in the State of Ohio or if working in WV will need a WV license as a Registered Nurse. BLS required upon hire. ACLS within 1 year of employment required. Special Knowledge, Skills, Training: Verbal, clerical, numerical, and manual dexterity, color vision and near visual acuity, proper lifting techniques and body mechanics. JOB DESCRIPTION SUMMARY The registered nurse plans, organizes and directs hospice care and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Patient Care 1. Completes an initial, comprehensive and ongoing comprehensive assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es). 2. Provides professional nursing care by utilizing all elements of nursing process. 3. Assesses and evaluates patient’s status by: A. Writing and initiating plan of care B. Regularly re-evaluating patient and family/caregiver needs C. Participating in revising the plan of care as necessary 4. Initiates the plan of care and makes necessary revisions as patient status and needs change. 5. Uses health assessment data to determine nursing diagnosis. 6. Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process. 7. Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care. 8. Counsels the patient and family in meeting nursing and related needs. 9. Hospice On Call RN

Serenity Hospice provides caring and compassionate hospice care services to patients and families throughout New Jersey. Our mission is to alleviate pain and symptoms; foster a state of calmness and harmony; and encourage an appreciation of family, friends, and life experiences. Serenity Hospice is looking to add a dedicated and compassionate On-Call Registered Nurse to delivering exceptional care, comfort and support to patients as needed. They will develop plans for individual needs, perform regular check-ups and communicate with family members as needed. Responsibilities: Respond to manage distress calls as needed. Provide nursing care, emotional support and education to patients and families. Complete an initial assessment of patient and family to determine hospice care needs Regularly re-evaluates the patients nursing needs Use assessment data to determine care plan needs Collaborate with the entire healthcare team, patient and family, to help direct the work of the team to provide care that is continuous and well-coordinated RN counsels the patient and family in meeting end of life needs Registered Nurse initiates appropriate preventative nursing procedures Communicates with the physician regarding patient's needs and reports any changes in the patient's condition Supervises the Hospice Aides and LPN's Requirements: Candidate must be able to work nights including weekends. Candidate must be able to work on-call schedule (one week on, one week off) Current NJ RN License in good standing Hospice Experience Preferred Willing to travel to the assigned areas Must have reliable transportation with liability insurance Strong communication and interpersonal skills.
