Job Title/Position : Hospice On Call RN Reports To : Clinical Director Offering Competitive Pay! JOB DESCRIPTION SUMMARY : The On-Call Hospice RN is responsible for responding to all of the Hospice patient census and their families, as well as handling any referrals or community inquiries outside of office hours every other weekend. The on-call nurse will provide urgent clinical and psychosocial interventions for patients and their families, skilled pain and symptom management and collaboration with providers within and outside of the hospice team to maximize positive patient outcomes. The work of the On-Call Hospice RN provides considerable consideration for independent assessment and judgement in crisis situations, utilizing and consulting with the Hospice Medical Director and administrator on call as indicated. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES : Consistent availability by telephone and email during assigned shifts. Conducts urgent physical and psychosocial assessments of the patient and family during after hours and determines appropriate interventions. Provides and maintains a safe environment for the patient. Provides professional nursing care by utilizing all elements of the nursing process. Provides skilled nursing care focusing on pain control and symptom management. Provides interim guidance to the patient and family regarding diagnosis, medications, treatments and progress. Documents patient care thoroughly and timely. Completes, maintains and submits accurate and relevant clinical documentation regarding patient's condition and care given. Records pain/symptom management changes/outcomes as appropriate. Communicates and collaborates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required. Works closely with members of the IDG to provide continuity of care and support for the patient and family. Collaborates with vendors to obtain DME, supplies and medications when needed after-hours. Collaborates with the hospital and other community residential staffing to adjust interventions and plans of care as needed. Conducts after hours hospice admissions as necessary. Participates in mandatory meetings as required. Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-services. Fulfills the obligation of requested assignments. Performs other job-related duties as assigned. POSITION QUALIFICATIONS : Understands and is committed to the hospice philosophy of care. Functions effectively as a team member. Holds a comprehensive knowledge of medication, treatment and therapies for pain and symptom management. Visits patients wherever they call home. Consults with physicians, other team members and administrative staff as indicated. Relates to and cares for people from all walks of life in a calm, courteous and professional manner. Maintains client confidentiality in accordance with HIPAA laws and company policy. Graduate of an accredited school of nursing. Maintains current unencumbered license to practice in the commonwealth of Florida as a Registered Nurse. Preferably has two years of experience in hospice, homecare, hospital medical-surgical or oncology nursing. Understands and works within state and federal laws and regulations and nursing scope of practice. Understands documentation requirements and is able to complete documentation in a timely manner. Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order. Must maintain current driver’s license and automobile insurance, providing own transportation. Reports must be submitted immediately when the reporting person has reasonable cause to believe that abandonment, abuse, financial exploitation, or neglect of a vulnerable adult has occurred. 6. Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers. Works in concert with the interdisciplinary group. 7. Provides and maintains a safe environment for the patient. 8. Assists the patient and family/caregiver and other team members in providing continuity of care. 9. Works in cooperation with the family/caregiver and hospice interdisciplinary group to meet the emotional needs of the patient and family/caregiver. 10. Attends interdisciplinary group meetings. Your team consists of Nurses, Nurse Aides, Social Workers, Spiritual Care Providers, a Volunteer Coordinator, a Bereavement Coordinator, and many other support functions to create a fulfilling experience for our patients and their families. You will be the hub for communication regarding patient care and will promote a positive working relationship with the family, physician, and others involved in their care. Position Responsibilities Directly supervises the interdisciplinary team. Responsibilities include interviewing, hiring, training and terminating (with administrative approval) all interdisciplinary team members; planning, assigning, and directing work; appraising performance; rewarding and disciplining colleagues; addressing complaints and resolving problems. Reads and incorporates into practice the requirements of the Hospice Conditions of Participation for Medicare, applicable accreditation standards, and state or other rules and regulations as applicable to the program. Oversees the consultative process between the Medical Director and the members of the Interdisciplinary Team (IDT), assists in requesting appropriate care orders and attendance to identified patient needs if patient's attending physician is not available. Attends/Leads Interdisciplinary Team (IDT) meetings, ensuring: Patient care needs are identified and discussed with the full complement of the IDT. All required members of the core team are present and engaged in the IDT meeting. Required documentation is completed during and at the conclusion of the IDT meeting to capture the discussion and outcomes of the meeting. Patient re-certifications are discussed and completed in a timely manner. Documentation of the patient-centered plan of care and the implementation of interventions for patient care including changes in level of care, medication changes, changes to visit frequency, or need for emergency transportation. Participates in the development and update of patient care policies and emergency procedures. Ensures hospice services are available 24/7. Follows state regulatory requirements for supervision of nursing services. Manages all members of the IDT including aspects of scheduling, productivity, mentoring, pay practices, time keeping, performance reviews, and colleague support. Models desired behaviors including timeliness, punctuality, attendance, collaboration, open communication, and equitable treatment. Actively collaborates regarding QAPI activities and provides feedback and input into Performance Improvement. Participates in performance improvement programs. Promotes hospice philosophy. Upholds a personal philosophy of integrity and commitment; communicates proactively when schedule changes are needed. Analyzes and reports on quality metrics including Hospice Item Set, CAHPS, and symptom management outcomes. Establishes departmental goals consistent with Moments Hospice goals and objectives. Supervises patient and family care as specified by the plan of care, assessing appropriateness, continuity, service, and quality of care. Models and facilitates communication between colleagues including medical records, business office, intake, clinical team, and leadership. Participates in care delivery for patients as needed including conducting admissions, recertification visits, routine visits, and discharge visits. Manages the care for a caseload of patients if needed to support ebb and flow of census changes. Participates in after-hours shifts as needed including primary and back-up on-call. Serves as the administrator on call when scheduled. Make and/or delegate post-admissions calls. Directs responsibility for the management of Pharmacy, Medical Supplies, Durable Medical Equipment, Mileage and Labor for program to meet budget expectations. May be designated the administrator or alternate administrator for the licensure of the program based on state requirements. Ensures maximum utilization of resources. Promotes a positive and collaborative culture. Performs other duties as assigned. * Education and/or Experience Registered Nurse with minimum of three years in Healthcare required, preferably in Nursing Facility, Home Health or Hospice setting. Bachelor's degree strongly preferred. Experience in leadership or management strongly preferred. Organizational Mission/Federal and State Regulations and Accreditation Standards 1. Services are available in accordance with organizational mission and core values. 2. Care and services provided by organization personnel and contracted organization personnel are coordinated and integrated and in accordance with local laws, regulations and accreditation standards. Responsible for the recruiting, retention and development of field staff. 1. The Recruitment and retention of employees for his/her respective site. 2. Responsible for arranging and implementing the onboarding/orientation and 90-day performance evaluations. 3. Responsible for timely annual performance evaluations for all direct care staff. 6. Responsible for ensuring the adherence of ongoing monthly educational requirements of all staff. 7. To ensure that exit interviews are conducted in accordance with organizational policies, for any member of the Clinical Team exiting the organization and that this information is submitted to the QA department for tracking and trending analysis. Budgeting/Productivity 1. Responsible to ensure that the daily/weekly/monthly productivity requirements for each direct care giver are met and monitors this report. 2. Monitors Key Performance Indicators on a periodic basis and resolves concerns in conjunction with other related departments. 3. In conjunction with Executive Management is responsible for the review of all pharmacy and DME related bills/expenses, investigating any variance and documenting the reasons. 4. Is responsible to review and approve all vendor bills in a timely manner. Provision of Quality Care 1. Is responsible for ensuring the on call schedule to meet the patient's needs and organizational policy. 2. Participates in rotating Administrator on Call. 3. Is responsible for the staff schedule to ensure the plan of care is met and that quality services are provided in accordance with each patient's plan of care. 4. Is responsible to document, investigate and report all alleged complaints in accordance with organizational policy, and communicate with the Compliance/QA department any adverse results. 5. Responsible to review and take the necessary corrective action of organizational identified quality measures for improvement of patient outcomes in conjunction with the QA department. 6. Responsible to review and take the necessary corrective actions of ongoing measurement of site-specific Performance Improvement Plan(s). 7. Responsible to review and take the necessary corrective action of chart audits for patients with long LOS 8. To ensure the timely reporting and review with staff the trending of all Adverse events. 9. Responsible to review and take the necessary corrective action to of IDG documentation 10. Responsible to review and take the necessary actions to ensure the timeliness of documentation from the clinical team. 11.
