Additional Information About the Role Monday-Friday 8-1630, weekends based on department needs Conducts new patient interviews and assesses patient's function health and educational needs. Monitors patient's lab values, test results and progress. Provides patient/family education. Preferred Qualifications Role Purpose Acts as a patient advocate by providing education, support and interventions throughout the continuum of patient care including pre & post hospitalization. Performs duties by working with physicians to identify and carry out treatment plans. Responsibilities Works with the Medical Director and other physicians to identify and carry out the treatment plan. Acts as a Patient Advocate helping to navigate patients through the diagnosis, treatment and rehabilitation process. Serves as a resource to assist patients and families, other professionals and the community. Provides education for the community and referring physicians. Facilitate clinical protocols established by Medical Director(s). BJC has determined this is a safety-sensitive position. Practicing excellent hand washing technique - Wash or gel my hands when going into and coming out of patient rooms and between glove use, especial when it relates to dressing changes, suctioning, etc. Change dressings when they need to be changed now rather than later and maintain all invasive lines as directed by policy. Vaccinate per policy. Notify my supervisor if I see anyone or anything that puts my patient at risk.
