Participates in the identification of resident problems that require nursing intervention Evaluates residents for any decline of condition, signs of change in condition, weight loss, decline in ambulation, change in ADLs, development of skin breakdown, etc. Notifies appropriate staff per facility protocol of the need for evaluation and informs staff on any changes in assessment when appropriate Confer with residents and families on any issues relating to care and services Make referrals to other departments when appropriate Complete employee and resident incident reports and complete follow-up investigation forms Maintain accountability for controlled substances by reviewing inventory medications with another licensed nurse and notifying supervisors of any discrepancies Performs routine charting duties in accordance with established charting and documentation policies and procedures Supervise the CNAs, LPN’s and the unit clerk in regards to work procedures, expediting workflow, evaluations, counseling and discipline Performs and monitors the appropriate transfer of residents, including the use of transfer equipment. Performs other related job duties as required. Qualifications: Must have an active license as a RN Nurse (RN) in the State of New York. Graduate from an accredited School of Nursing Knowledge of practical nursing theory and practice. Ability to follow oral and written directions. Prepare and maintain records, charts, and reports. Provide assessment of readmissions to determine cause and develop plan. Track patient progress and communication with providers in on-line database. Review and interpret claims data from CMS (provided in user-friendly format Communicate with patient on an on-going basis to improve healthcare outcomes.(Medication review, education, MD follow-up, etc.) Assists with pre op phone calls and participation in Joint replacement education class. Presents data via google slides, etc. as requested to key stakeholders within the organization for review including physician and hospital leadership. Meets with partnering SNF's regulartly onsite to review LOS data, treatment plans, outcomes or discuss challenging patients. Assists with high lace post discharge phone calls, intervenes as necessary to prevent readmission. Provides coverage when needed in the Continuous Care Department as directed by Continuous Care leadership Other duties as assigned Education Qualifications Bachelor's Degree BSN Required and Experience Qualifications 3-5 years of experience in an acute care setting Required and Disease management and discharge planning experience Preferred Knowledge, Skills, And Abilities Analytical and problem solving along with excellent verbal and written communication skills.
