Registered Nurse (RN) Salary: $48 an hour (Based on Experience) Registered Nurse (RN) Tired of the same old nursing home routine? We are taking a very fresh approach to skilled nursing. Follow facility procedures/protocols for admitting residents transferring to hospitals/clinics/outpatient visits and completion of appropriate documentation based on established guideline. Makes rounds on all resident rooms and ensure that the L.P.N.’s, C.N.A.’s and/or others under his/her supervision receive their resident reports, has their assignments and break times scheduled. Assist the L.P.N. in establishing routines which facilitate a high level of nursing practice. Receives and give reports from and to outgoing and incoming shifts. Count or ensure the counting of controlled drugs with outgoing and incoming shifts. Coordinates/Prepares MDS schedules for quarterly, annual and significant change assessments. Complete MDS Booklet as per schedule (Nursing Sections). Reviews and signs off on all completed MDS and RAP Summary. Develops care plan that demonstrate knowledge of criteria to ascertain resident specific needs based on health care problem(s). Makes needed revisions and updates to the care plans. Ensures that care plan is carried out which demonstrates familiarity with criteria to establish priorities specific to anticipated outcomes. Monitors the provision of care for the residents in his/her assigned unit. Interprets philosophy and objectives of nursing service to residents, families and others. Identifies resident/family learning needs based on the discharge care plan as applicable and carries out appropriate teaching required. Demonstrates ability to coordinate and collaborate with resident/family and other members of the comprehensive health care team. Monitors the administration and accurate recording of medications used. Investigates and follow through on medication errors. Notifies physicians of pertinent observations and reactions, incident and abnormal laboratory and x-ray results as required and documents same promptly. Transcribes physician orders timely and ensure orders are implemented as appropriate. Reports all resident, visitor and work-related accidents/injuries and demonstrates appropriate follow through by investigating, documenting and processing incident reports. Accepts guidance, supervision, and criticism if needed. Practices in compliance with the Nurse Practice Act and regulatory agencies regulations. Assures that a stock level of medications, medical supplies, equipment, etc. are maintained at adequate levels at all times. Assume the authority, responsibility and accountability of directing your nursing shifts. Other responsibilities as deemed necessary and appropriate or as may be directed by the Director of Nursing Services or Administrator. Registered Nurse (RN) Qualifications/Requirements: Graduate of an accredited School of Nursing M.S./B.S. or equivalent experience preferred Licensed to practice in NY Nursing Experience which one has acquired clinical competence and strong leadership skills. Demonstrates an understanding of and appreciation of the aging process. Ability to be self-directive in delegated areas of responsibility. The Nurse functions under the guidance of their State Licensure within the facility in accordance with State and Federal regulations. We are seeking a Nurse to join the Monte Nido team. Responsibilities Include: Essential Duties and Responsibilities Initiates the admission process by obtaining provider orders upon patient arrival at a residential facility and completing the initial nursing assessment within 24 hours, including assistance with medical intake (patient history and physical). Performs vital sign monitoring, including orthostatic vitals and blind weights as ordered by the provider; conducts patient assessments and documents findings in detailed nursing notes. Monitors and documents patient progress or regression in accordance with established policies and procedures. Assists providers in facilitating weekly patient consultations. Offers education and support to help patients develop the skills needed for safe self-administration of medications after discharge. Ensures accurate placement of medications in the designated medication box and updates the medication administration record (MAR) accordingly. Supports patients in obtaining medications from the pharmacy, including facilitating initial medication orders. Coordinates and facilitates the collection of laboratory specimens as ordered, ensures timely receipt of results, and communicates findings to the medical team for appropriate follow-up. Other Duties and Responsibilities Demonstrates knowledge of medication actions, side effects, and contraindications, and is responsible for researching unfamiliar medications prior to administration to ensure safe and informed care. Collects and records data in a comprehensive, accurate, and systematic manner. Responsible for maintaining patients’ records updated in electronic system. Supports a therapeutic milieu by monitoring patient behaviors, fostering a recovery-focused environment, and collaborating with the treatment team to reinforce structure, safety, and consistency. Communicates relevant information obtained from patients or families to the interdisciplinary team to support individualized care planning and treatment continuity. Coordinates with the Primary Therapist on any insurance review information needed. Works collaboratively with medical and psychiatric teams to provide integrated nursing care in support of eating disorder treatment goals. Participates in discharge planning with the medical and psychiatric teams to ensure continuity of care and safe transition from eating disorder treatment. Attend monthly nurse-led meetings and other team meetings as they arise to support effective communication and collaboration. Provides health education to patients through individual or group sessions when appropriate and documents sessions clearly, accurately, and concisely using the designated form. Assist patients in the coordination of their outpatient appointments as needed. Supports the informed consent process by helping patients understand procedures and treatments under the direction of the provider. Knows Baker Act (Florida Only) Delivers clinical nursing care in a safe, competent, and professional manner in accordance with established standards and protocols. Adheres to established standards of nursing practice and organizational policies. Completes required onboarding and annual training within designated timeframes and attends monthly clinical services training or other training as scheduled, in accordance with organizational policies and procedures. Demonstrates professionalism and serves as a positive role model for patients and staff within the therapeutic environment. Promptly reports suspected patient rights violations by established protocols. In an emergency or declared state of emergency, it may be required to stay on-site as a part of the emergency team until the emergency has ended. Performs other tasks, as assigned. Qualifications: Graduate from an accredited school of nursing is required. A minimum of one year of experience, preferred. Valid Nursing licensure required. Virtual Programs - Multistate Nursing license, preferred. CPR/First Aid certification required.
