Develop objective clinical assessments that support appropriate care planning and service recommendations. Educate patients and family members about the assessment process and available home care services. Collaborate with interdisciplinary care teams, case managers, and healthcare providers to ensure continuity of care. Maintain compliance with New York State Department of Health regulations, agency policies, and documentation standards. Travel throughout assigned service areas while managing a flexible schedule. Qualifications Active and unrestricted New York State Registered Nurse (RN) license. Current UAS-NY Certification is strongly preferred (or willingness to obtain certification if applicable). Minimum of one (1) year of nursing experience required. Home Health, Managed Long-Term Care (MLTC), Case Management, or UAS Assessment experience preferred. Strong clinical assessment, critical thinking, and decision-making skills. Excellent communication, documentation, and interpersonal skills.
