Complete patient intake for procedural abortion and family planning services as needed. Perform transvaginal and/or abdominal ultrasounds Perform in-house labs such as hemoglobin, RH factor, rapid HIV tests and pregnancy tests. Collect blood, urine, pharyngeal and rectal samples for testing and order labs as instructed by an Advanced Practice Clinician or physician. Complete sedation screening on all patients who elect moderate sedation; screening includes but is not limited to review of vital signs, BMI, oxygen saturation, ASA classification, risk of obstructive sleep apnea, physical exam of head and neck, and auscultation of heart and lungs. Initiate peripheral intravenous lines and withdraw blood as needed. Elicit and answer questions regarding the surgical abortion procedure, risks, and aftercare. Elicit and answer questions regarding return of menstruation and birth control use/side effects. Inform patients of next steps and expectations in their surgical abortion appointment. Review and sign informed consent forms for surgical abortion with patients before administering cervical preparation medication. Assist the abortion provider throughout the surgical abortion including room set-up, rooming of patient, monitoring of patient vital signs, comforting the patient, and responding to emergencies. Receive patients in the recovery room and assist in transferring patient from the procedure room to the recovery room as needed. Create a warm, comforting environment in the recovery room. Supervise patients in the recovery room including but not limited to taking vital signs, assessing pain level, evaluating bleeding, educating, and using clinical judgement to escalate concerns to the abortion provider. Provide education to patients regarding all medications to be administered, including their desired effects, side effects and risks. On an Average Day You Will: (includes but not limited to) Relate information to the DPCS, ED and Community Education Director of initiatives that promote hospice access in the hospice community Ensure the educational needs of the community related to hospice care and services, are assessed and met on an ongoing basis Establish and monitor community, customer, payer and patient perceptions of Bristol Hospice as a high-quality provider of hospice services in the community Facilitate communication with agency and referral sources as needed to promote the continuum of care Maintain comprehensive working knowledge in the field of Hospice educational resources and share information with appropriate organizational personnel Facilitate the hospice referral process Serve as a resource to review and consult available patient information related to the case, to determine hospice care needs and criteria Coordinate and communicate with hospice agency leadership and referral sources to promote care coordination and a timely admission process Complete hospice consults/evaluations and admissions Ensure coordination and communication with referral sources and hospice agency leadership Complete initial assessments of patients and families to determine hospice needs Facilitate and complete admissions as indicated Provide a complete physical assessment and history of current and previous illness(es) of referrals Work with the intake department so they can serve as a liaison between facility, physicians, hospitals, skilled nursing facilities, community and hospice programs, with coordination of referrals/admission process and services Coordinate with the ED and DPCS regarding the need for planning, implementation and evaluation of in-service and continuing education programs May be required to actively participate in the Quality Assurance Performance Improvement (QAPI) process, as directed by ED or DPCS May be required to function as an RN Case Manager and maintain an assigned patient case load as determined by the ED or the DPCS Provide professional nursing care by utilizing all elements of the nursing process Use health assessment data to determine nursing diagnosis May be required to participate in IDT meetings and work with the IDT to determine the patient/family plan of care, as needed Provide patient/family education related to the patient/family plan of care All other duties as assigned Requirements: Must have a current RN license in the state Must have a minimum of one (1) year of experience as a professional nurse within the last three (3) years or have a bachelor’s degree in nursing from a program accredited by the National League of Nursing Must be able to travel 20% of the time, often without notice Must be flexible in work hours Must possess the willingness to maintain comprehensive working knowledge regarding information systems and applicable software programs Our Ideal Candidate Has the Following Skills and Knowledge: Ability to read and follow written instructions An Understanding of hospice philosophy and comfortable providing specialized care to the terminally ill Ability to work autonomously with little direct supervision Successfully demonstrate tact, patience and good personal hygiene Ability to communicate effectively Strong organizational skills Understanding of plan of care documentation Flexible in work hours to ensure the patients receive care as provided in the plan of care Ability to empathize with the needs of the ill, injured, frail and impaired Comfortable addressing issues of death/dying Calm demeanor when entering the patients home We Got the Perks: Tuition Reimbursement PTO and Paid Holidays Medical, Dental, Vision, Life Insurance, and more HSA & 401(k) available Mileage Reimbursement for applicable positions Advanced training programs Passionate company culture committed to the highest standard of care in the hospice industry Join a Team that embraces the reverence of life!
