We are seeking a dedicated Hospice Nurse with a strong commitment to quality patient care. To succeed in this role, you must possess excellent clinical nursing and customer service skills. You will be responsible for completing a detailed initial assessment of the patient, the family, and the facility/home environment to determine specific care needs, monitor and regularly re-evaluate the patients nursing needs, and initiate the appropriate plan of care as well as making necessary revisions as patient status and needs as they change. THIS POSITION IS EVERY WEEKEND FROM 7AM TO 7PM FRIDAY-SUNDAY. IT WILL COVER ALL OF NORTHERN ILLINOIS/NORTH SHORE/NORTHWEST SUBURBS. Hourly Pay Range: $37.25 - $57.74 - The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors. Position Highlights: Position: RN - Resource Team/Float Pool Location: Arlington Heights, IL Status: Per Diem- Level 1 Hours: 7:00am -7:30pm Per Diem Commitment: Level 1 -must be able to work at least 48 hours every 6 weeks, including 24 hours of weekend time. Required Travel: No What you will do: Provides patient assessment and collection of health data involving patient, family and other health care providers. Coordinates patient plan of care to assure interventions are consistent, safe, timely and appropriate. Communicates clearly and effectively with all customers, in person, on the phone and in writing. Individualizes patient and family teaching based on a thorough assessment to meet their specific learning needs. Participates in evaluating quality and effectiveness of nursing practice by participating in quality improvement initiatives. Maintains professional knowledge in current nursing practice standards, laws and regulations.
