Enters date and time of entry and signs name in legible manner on patient's chart. Completes Nursing Admission Assessment 24 hours after admission. Documents patient's response to interventions and plan of care on the Progress Notes Performs vital signs and pain score at least every four hours. Completes Transfer Assessments on patients transferred out of the unit and Perioperative Assessments on patients scheduled for surgery or procedures. Completes Frequent Observations and the Intake/Outtake Record according to established protocols. Initiates and completes health care referrals in collaboration with the interdisciplinary team. * Provides patient education. Identifies patients learning needs upon admission.Informs and educates patient and family to enable participation in their care and decision making. Overview Carries out patient care responsibilities with emphasis on decision-making related to the steps in the nursing process to include assessment, diagnosis, and identification of expected outcomes, planning, implementation and evaluation. Provides competent, safe and compassionate care based on established BronxCare Health System Patient Care and Nursing Standards. As a member of the interdisciplinary healthcare team, formulates the initial plan of care with the provider (physician or nurse practitioner) and collaborates with them to implement a comprehensive patient-centered plan of care to achieve expected outcomes; utilizes principles of assignment and delegation consistent with the NY State Nurse Practice Act. Responsibilities Provides professional nursing care to patients within unit.
