Job Description The Hospital at Home Nurse Navigator is responsible for providing high-quality discharge planning, care coordination, and follow-up care for patients involved in the UChicago Medicine’s Acute Hospital Care at Home model. Comer Emergency Department is a 21-bed, Level 1 Pediatric Academic Trauma Center dedicated to providing high-quality, evidence-based care to the children of South Side of Chicago and beyond. With 2 trauma bays, 1 special procedures room and a dedicated resuscitation bay, we are equipped to handle the most critical situations. The Adult Emergency Department consists of 55 patient care rooms including a rapid assessment unit, six trauma/resuscitation suites and separate rooms for infection isolation, care of immune compromised patients, and psychiatric consultations. In the general care area, 45 rooms are fully equipped with critical care monitoring capabilities. This position is a 100% onsite opportunity . You will need to be based in the greater Chicagoland area. A staff nurse is a registered professional nurse (RN) who manages patient centered care that is outcome directed. Patient centered care principles and professional nursing practice standards provide the foundation for the nurse to assess, plan, implement and evaluate the care provided.
