The Home Care Primary Nurse is responsible for the general oversight, assessment, planning, and construction of the nursing care delivered to the participant. The home care primary nurse may delegate some direct care, data collection, and nursing assessment responsibilities to other nurses and nursing assistants on the team but retains responsibility for ensuring that interventions are carried out in accordance with the participant’s care plan and the orders of the participant’s medical providers. The RN is responsible for the delivery and documentation of quality patient care in accordance with departmental policies and procedures, and in collaboration with other members of the Interdisciplinary Team. ESP RNs may provide care in the PACE Day Health Center or in participant’s homes. Essential Duties and Responsibilities: Assess a patient through obtaining history, performing appropriate tests, making or collecting observations from the participant and caregivers, and consulting with other members of the IDT. Prepares and presents a patient for rapid provider review and treatment. Arranges for or directly implements care plan interventions and testing, treatment, or other provider orders for a panel of participants. Oversees delegated tasks to ensure completion, and monitor outcomes, and make adjustments to care plan as needed. Safely and effectively administers medications, tests and treatments as ordered. Manages the refills of all patient medications with appropriate authorization by the provider. Teaches patients and families health care regimens. Explains and provides information regarding disease and wellness, tests, results, medications, diet, exercise, and other aspects of the plan of care to patients. Qualifications and Requirements: Baccalaureate degree in nursing preferred. Current Massachusetts license in nursing. Current BLS certification required. Minimum of 2 years experience in ambulatory, home health, hospital or sub-acute setting preferred, equivalent training or related experience considered. A minimum of one year experience working with a frail or elderly population. Skills/Abilities: Basic physical assessment and triage skills. Ability to communicate assessment and triage findings efficiently, effectively and accurately in verbal and written formats. Ability to communicate effectively with professional and non-professional staff, outside providers, participants and families in person and by phone. Bilingual a plus. Excellent customer service skills. Flexibility, initiative, creativity and excellent organizational skills. Basic computer skills including experience with word processing and email.
