Every other weekend Scope of Work Plans and prioritizes care for individuals and population of patients, focusing on strategies that will promote optimal health within populations. Demonstrates expertise, current knowledge in nursing care and management of a caseload of patients of varying complexity and seeks to improve patient, family and health systems / community outcomes through the application of educational concepts / skills and preventive care in a managed care environment. Assesses internal and external referrals to identify patient/significant others’ needs, level of intensity, insurance benefits and other patient resources. Develops plan of care and makes recommendations to PCPs, specialists and other members of the health care team regarding care management strategies, identifying strategies to maximize continuity of care across the continuum. Assesses the educational needs of patients, families and members of the health care team and develops and implements appropriate teaching strategies or makes appropriate referrals. Communicates and collaborates with patient/significant others/providers/payers to coordinate services that improve access to appropriate services across the continuum of care and which promotes optimal health in a cost-effective manner. Measures patient care outcomes, interprets reports and analyzes data trends for groups of patients. Documents patient data, plan, interventions and outcomes according to department guidelines. RN Infusion Weekends Part Time

About the Position Mixed Weekend/Weekday Infusion RN Role in the infusion clinic at Lemmen Holton Cancer Pavilion. Serving the oncology and general populations by providing a wide range of infusion services and medication as well as blood product transfusion. Looking for candidates for whom weekend work is preferred and who want to work on a high functioning team. Recommend only candidates who are looking for a team nursing work model and a relational model of care. Some orientation days may need to be completed on weekdays as the weekend clinic is a new initiative this year. Would recommend to candidates who are seeking to contribute to a new service and for whom the weekend requirement work well with their work-life balance needs. Weekend shifts are abbreviated, typically 6 hours from 7:30 a.m. to 2:00 p.m. so evenings would be free. Psych RN - $10K Sign On Bonus

Responsibilities Forest View Hospital, located just seven miles southeast of downtown Grand Rapids, Michigan, is a private 108 bed psychiatric facility that serves children, adolescents and adults. We are licensed by the State of Michigan, fully accredited by The Joint Commission and we bring more than 45 years of experience to the evaluation, diagnosis and treatment of a wide range of behavioral health problems. As a part of our continued success and growth, we are seeking qualified applicants for a Psych RN. *There is a $10,000.00 Sign On Bonus Available with this Position* The Psych RN provides nursing care to patients on each clinical unit as assigned, utilizing the nursing process. Patient population serviced includes both adolescents and adults. Patients are hospitalized for a broad variety of psychiatric disorders, including mood, anxiety, and thought disorders. Patients may also require treatment in specialty programming for trauma, eating disorders, and dual diagnosis (both substance abuse and psychiatric issues). This position actively supports, promotes and works to fulfill the Mission, Vision and Values of Forest View Hospital using our Service Excellence program. This is a Fulltime, 36 hours a week position (12 Hour Shifts- 7:00pm-7:30am or 7:00am-7:30pm). Essential Job Responsibilities: Promotes a warm and welcoming environment when paired with newly hired RNs, providing training, guidance, and support that is consistent with patient care, safety standards, and policy and procedure. Responds to questions in a respectful manner. Reports concerns promptly when identified. Acts as back-up point of contact for direction of other unit staff members when Charge is unavailable Provides leadership to other personnel in crisis behavioral management Utilizing the nursing process, the RN develops an initial nursing treatment plan for each admitted patient. Competent in assessing, planning, and delivering care for patients with co-occurring disorders, eating disorders, and trauma backgrounds. Directs and participates with other personnel in maintaining a safe and therapeutic milieu. Assists with discharge planning and works with patient at time of discharge. Provides leadership to other personnel in crisis behavioral management. Current RN licensure in the state of Michigan. Demonstrates clinical competency in providing general nursing care. Basic understanding of psychiatric nursing theory CPR/Handle With Care certification preferred.
