Registered Nurse, Primary Care - Full-Time, Days

This position is a 100% onsite opportunity. You will need to be based in the greater Chicagoland area. Our Kenwood location offers exceptional primary care for your most basic and pressing health needs. In addition to general primary care for adults of all ages, this site is equipped with primary care experts and integrated care services specifically for people of all ages. A Registered Nurse, UCM Care Network, is responsible for providing professional nursing care for patients including assessment, treatment, care planning, and medical care evaluation by following established standards, protocols and practices. Assists physicians and staff in coordinating patient care. Functions as primary liaison between patients, staff and physicians. This position will be located in a community-based physician practice. Essential Job Functions Obtains patient consent for care and ensures patient confidentiality Obtains and documents health history, information on chief complaint, vital signs, and health risk factors and assesses disease condition Provides patient care based on practice guidelines, standards of care, and federal/state laws and regulations Executes prescribed treatments and medical interventions, administers prescribed medications, and monitors and documents treatment progress and patient response Delegates as allowed under the Nurse Practice Act. Refers to/consults with physicians, other health providers, and community resources to prevent/resolve problems or concerns Educates patients and families about health status, health maintenance, and management of acute and chronic conditions Documents patient assessment and intervention data using established medical record forms/automated systems and documentation practices Participates in multidisciplinary teams to improve patient care processes and outcomes Tracks quality assurance data and monitors for acute and chronic care management Maintains/reviews patient records, charts and other pertinent information Posts tests and examination results Responds to/refers incoming patient treatment-related phone calls Maintains patient confidentiality Performs other duties assigned Required Qualifications Must be an actively licensed Registered Nurse in the state of Illinois Minimum of one year professional nursing experience Completion of nursing training from an accredited school of nursing Active BLS certification from the American Heart Association (AHA) Preferred Qualifications A Bachelor's degree in Nursing Prior clinic experience Position Details Job Type/FTE: Full-Time 1.0 FTE Shift: Days - 9 am – 5:30 pm Work Location: Onsite - Kenwood Unit/Department: Primary Care CBA Code: Non-Union

This position is a 100% onsite opportunity . You will need to be based in the greater Chicagoland area. At UChicago Medicine, we bring the world’s most innovative approaches to patients with all conditions, from common to complex. This department has an excellent team that is knowledgeable about cancer care and eager to learn, teach and share new treatments for our patients and families. This unit provides care for patients with array of cancer diagnosis. The main population is solid tumor patients that are in for symptom management, chemotherapy treatments, supportive oncology and palliative care. We care for the whole patient and family to ensure they are knowledgeable about their diagnosis and treatment. It is a very busy unit and you will learn a lot about holistic care of the patient. You will have the ability to work with physicians that are constantly pushing the bounds of possibility forward, conducting groundbreaking research and providing lifesaving, personalized patient care. A staff nurse is a registered professional nurse (RN) who manages patient centered care that is outcome directed. Patient centered care principles and professional nursing practice standards provide the foundation for the nurse to assess, plan, implement and evaluate the care provided. The staff nurse assumes the primary responsibility and accountability for all patient care personally provided and that provided by those directly under her/his supervision. Essential Job Functions Utilizes appropriate assessment skills to determine patient needs. Develops a plan of care for those patients whose needs do not require the expertise of a Case Manager or collaborates with the Case Manager to develop a plan of care for specified patients. ADN nurses receive 100% tuition reimbursement towards a BSN degree from an accredited program.
