Responsibilities Perform resident assessments routinely and as needed for condition changes, hospital returns, or unscheduled events. Accurately observe, assess, and report changes in resident conditions to appropriate personnel. Ensure timely completion of assessments and CAA’s by interdisciplinary departments, including Social Services, Activities, and Dietary. Complete MDS assessments, quarterly reviews, CAA’s, and related care plans. Initiate, update, and maintain care plans based on resident needs identified through assessment data. Prepare and distribute MDS schedules to interdisciplinary team members. Transmit MDS data to the state weekly (or as required) and maintain accurate transmittal documentation and logs. Qualifications Current Registered Nurse (RN) license in the state of Pennsylvania. Minimum 1 year of MDS Assessment Coordinator experience in a long-term care setting.
