Duties Description Independently performs the duties below: Conducts nursing assessments; and obtains, records, and updates individuals’ health information and vital signs, using standard instruments and procedures. Monitors inventories and ensures that standards for medications, solutions, supplies, and equipment are maintained to meet nursing care needs. Provides clinical supervision and instruction to Graduate Nurses, Licensed Practical Nurses, and direct care staff in completing nursing care tasks. Delegates responsibility for appropriate nursing activities, skills, and/or procedures to nursing and direct care staff; and makes clinical rounds to assess individuals’ nursing care needs and observe and evaluate the provision of nursing services. Acts as liaison to outside agencies and providers to ensure continuity of care and the necessary follow-up for individuals; advocates for individuals’ needs; and may provide instruction to providers on individuals’ discharge plans. May perform the full range of supervisory duties over Graduate Nurses, Licensed Practical Nurses, and direct care staff; provide information to Registered Nurse Supervisors; and prepare or contribute to the preparation of reports and performance evaluations for lower-level staff involved in nursing care activities. Duties Description Independently performs the duties below: Conducts nursing assessments; and obtains, records, and updates individuals’ health information and vital signs, using standard instruments and procedures. Prepares, implements, and modifies nursing care plans; prepares individuals for and/or assists with examinations and treatments; and administers medications and nursing procedures to individuals, as prescribed Observes, monitors, and records individuals’ symptoms, conditions, and care, including diet and physical activity; and identifies and reports individuals’ responses to medication or changes in health status. Serves as the nursing representative on a treatment team; provides information on nursing needs; and consults and coordinates with treatment teams and other health care disciplines to assess, plan, implement, or evaluate individuals’ care. Makes clinical rounds to assess individuals’ nursing care needs and observe and participate in the provision of nursing services. Provides health teaching, counseling, and education to individuals, families, and other groups; and develops health improvement programs. Implements infection control protocols and procedures and provides related consultation and instruction to staff. Maintains and updates charts, records, and other documentation, consistent with agency, State, and federal requirements; and may collect data and perform related activities to support medical research. Monitors inventories and ensures that standards for medications, solutions, supplies, and equipment are maintained to meet nursing care needs. Provides clinical supervision and instruction to Graduate Nurses, Licensed Practical Nurses, and direct care staff in completing nursing care tasks. Delegates responsibility for appropriate nursing activities, skills, and/or procedures to nursing and direct care staff; and makes clinical rounds to assess individuals’ nursing care needs and observe and evaluate the provision of nursing services. Acts as liaison to outside agencies and providers to ensure continuity of care and the necessary follow-up for individuals; advocates for individuals’ needs; and may provide instruction to providers on individuals’ discharge plans. May perform the full range of supervisory duties over Graduate Nurses, Licensed Practical Nurses, and direct care staff; provide information to Registered Nurse Supervisors; and prepare or contribute to the preparation of reports and performance evaluations for lower-level staff involved in nursing care activities. Additional Comments This title is part of the New York Hiring for Emergency Limited Placement Statewide Program (NY HELPS). HELPS Program titles may be filled via a non-competitive appointment. This means that you do not need to take an exam to qualify, but you do need to meet the minimum qualifications of the title. At a future date (within one year of permanent appointment), employees hired under NY HELPS are expected to have their permanent non-competitive employment status converted to permanent competitive status. You will not have to take an exam to gain permanent competitive status. Please note that starting salary for this title reflects the increased hiring salary, Downstate adjustment and geographic pay differential. Posted Hiring Salary includes the sum of the statutory hiring rate for PS&T Grade 18 ($66,951), Increased Hiring Salary ($9,196), Downstate Adjustment ($4,000), and Geographic Pay Differential ($35,000). Geographic Pay Differential and Downstate Adjustment are added to the job rate ($85,138). Home Health Nurse Manager - Mount Sinai Health- Full Time Days

Description The Home Health Nurse Manager is responsible for the management and oversight of clinical staff for the licensed home care agency and nursing staff supporting home infusion services under the guidance of the Director of Patient Services, S/he ensures that personnel issues are managed in compliance with Department of Health (DOH) regulations and contractual agreements. S/he works in collaboration with clinical leadership, administrative leadership, providers of care and referrers to ensure that the service maintains the highest standards of patient care and is responsive to the needs of the patient and family and the organization. Responsibilities Patient Experience 1. Supervises all clinical staff of Mount Sinai Home Health LLC home care agency and nursing staff supporting home infusion services. 2. Oversees the agency orientation for all newly hired clinical staff including introduction to agency policy and procedures, clinical skills training and assessment, documentation and EMR requirements and competency evaluations. 3. Ensures clinical staff adherence with written policies and procedures 4. Performs field supervision visits with all clinical staff on an annual basis at a minimum and remediation supervision visits as needed. 5. Monitors data regarding patient census staff productivity and collaborates with Director of Patient services to ensure financial stability. 6. Participates in the recruitment and selection of personnel. 7. Oversees the maintenance of patient clinical records and clinical staff personnel files. 8. Leads nursing practice using the tenets of the Professional Practice Model of Relationship Centered Care and the Care Delivery Model of Modified Primary Nursing for the promotion of patient experience. 9. Synthesizes quantitative and qualitative feedback regarding the patient experience and staff engagement to Nursing Management, staff and the Interdisciplinary Patient Care Team that leads to the development and implementation of an action plan. 10. Follows up on all patient concerns and complaints in a timely manner, provides service recovery as needed and escalates patient concerns as appropriate. 11. Demonstrates caring, respect, compassion, empathy and active listening through dialogue, body language and actions. Quality and Safety 1. Supports and leads a safe, therapeutic and efficient patient-centered care environment for professional practice. 2. Completes chart audits for quality and compliance monitoring 3. Demonstrates accountability for quality and safety practices and compliance and ensures maintenance of mandatory staff competency evaluations. 9. Fosters a professional practice environment through the facilitation of collegial interdisciplinary relationships, peer review, nursing research, innovation and joint practice arrangement through affiliating universities. Operations and Administration 1. Supports implementation of the mission, vision, goals and objectives of the agency, throughout the clinical service and individual patient care units. 2. Master’s degree in nursing or health related specialty preferred.
