Description The (Per Diem) Clinical Nurse is a Registered Professional Nurse who provides safe, competent quality care based on nursing theory and research to a designated group of patients and significant others. Summary The Registered Nurse provides direct patient care that is guided by the American Nurses Association (ANA) scope of practice. A Registered Nurse collaborates with patients, family/significant others, peers, physicians, and other members of the interdisciplinary healthcare team in the delivery of healthcare services to patients. The program combines daily in-home visits, virtual visits, and 24/7 remote patient monitoring to give eligible patients an opportunity to stay in the comfort of their own home while receiving their hospital care. This program will be designed and delivered by NYP providers and clinical staff, allowing NYP's best-in-class care across all measures of quality, service, cost, and innovation to be continued in the home setting.
