Summary Works with physicians and multidisciplinary team members to develop a plan of care for each assigned patient from admission through discharge. Assesses, creates, implements evaluates, and modifies timely discharge plans. Assesses, intervenes, and acts as a resource in medical-legal situations for patients and families. Enters nursing orders for their assigned patient caseload, in addition to those being handled by the social worker care coordinator, related to discharge planning and transitional care needs, as they are directly involved in patient care and coordinating the discharge process, including documenting necessary orders for post-discharge management. Assists patients and families in making difficult decisions, while honoring their preferences and values, in order to move the discharge plan forward. Collaborates with patients, family/caregiver, nursing, physician(s), and other members of the multidisciplinary team, creating consensus around issues of discharge planning. Ensures documentation in the electronic medical record is entered timely, is clear, complete, concise, and organized. Monitors clinically high risk and complicated cases and institutes necessary actions to promote quality care and appropriate integration with timely escalation as appropriate. Acknowledges and completes case management consults in a timely manner and identifies patients that meet the criteria for case management. Coordinates cases between health care providers and payors. Facilitation of precertification and payor authorization processes for medication. Facilitates the collaborative management of patient care across the continuum, intervening as necessary to remove barriers to timely and efficient care delivery and transition of care. Facilitates coordination between multidisciplinary team and patient/family/caregiver for care conferences when indicated. Develops treatment plan and discusses proposed course of treatment with patient’s attending physician, patient, family/caregiver, and other members of the multidisciplinary team. Ensures patient/family/caregiver/staff concerns are appropriately resolved in a timely manner. Identifies populations and patients at risk for re-admissions, establishes goals with patients and families, coordinates specific focused discharge interventions and services, makes contact with community partners for continuity of care. Works with clients and multidisciplinary team to identify discrepancies and barriers to health, wellness, and independence towards health equity. Improves understanding of access points for medical care resulting in decreased use of emergency resources, decreased hospital admission, re-admission, and unnecessary expenditures. Performs related duties as assigned. NICU Discharge Planner Follows all NICU patients to identify and coordinate inpatient, discharge planning and case management needs. Provides discharge education to patient families. Completes enrollment of qualified infants into the High-Risk Perinatal Program (HRPP) according to the Arizona Department Health Services criteria. Coordinates inpatient Retinopathy of Prematurity (ROP) Screening Program for at-risk NICU patients. Arranges discharge medical equipment, supplies, and medications and arranges or provides related training as indicated. Maintains Interdisciplinary Rounding List for discharge planning. Conducts discharge follow up calls for identified patients/families. Minimum Qualifications EDUCATION: Bachelor's degree in nursing or associate degree in nursing. EXPERIENCE: Two (2) years of nursing or case management experience. For NICU – Three (3) years of NICU nursing experience. LICENSURE OR CERTIFICATION: Current RN licensure permitting work in state of Arizona and basic life support (BLS) required. This all‐private room unit cares for the post‐surgical patient following vaginal and abdominal hysterectomy, bladder repairs, laparoscopic procedures, plastics, mastectomies/breast reconstruction, and women with medical‐surgical complications. The Women’s Care Unit also specializes in providing care to high‐risk antepartum patients, who sometimes spend weeks or months as an inpatient until the time of delivery. The all-female staff provides compassionate care to patients who have experienced a fetal loss or are placing a baby for adoption. Principal Responsibilities • Works in collaboration with the Interdisciplinary Team to assess the needs of the resident; Provides interdisciplinary schedule for MDS assessments and care plan reviews as required by governing agencies. • Ensures that the Interdisciplinary team makes decisions for either completing or not completing additional MDS, assessments based on clinical criteria as identified in the most recent version of the RAI User’s Manual. • Assist with coordination and management of the daily stand up meeting, to include review of resident care and the setting of the assessment reference date(s). • Complies with federal and state regulations regarding completion and coordination of the RAI process. • Monitors MDS and care plan documentation for all residents; ensures documentation is present in the medical record to support MDS coding. • Maintains current MDS status of assigned residents according to state and federal guidelines. • Maintains the frequent and accurate data entry of resident information into appropriate computerized MDS programs. • Completes accurate coding of the MDS with information obtained via medical record review as well as observation and interview with facility staff, resident and family members. • Attends interdisciplinary team meeting, quality assurance and other meeting in order to gather information, communicate changes, and maintain and update records. • Assists DON or designee with identification of a significant change, physician orders and verbal reports to assure that the MDS and care plan are reflective of those changes. • Prepares scheduling, notice of resident care planning conferences, and assists DON in communication of outcomes/problems to the responsible staff, resident, and/or responsible party. • Continually updating knowledge base related to data entry and computer technology. • Completes electronic submission of required documentation to the state database and other entities per company policy. • Corrects and ensures completion of final MDS and submits resident assessment data to the appropriate State and Federal government agencies. • Assigns, assists, and instructs staff in the RAI Process, PPS Medicare, Medicaid (Case Mix as required) and clinical computer system in relation to these processes. • Maintains confidentiality of necessary information. • Other duties, responsibilities and activities may change or assigned at any time with or without notice. Qualifications • Graduate of an approved Registered Nurse program and licensed in the state of practice, required. • Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred. • Excellent knowledge of Case-Mix, the Federal Medicare PPS process, and Medicaid reimbursement, as required. • Thorough understanding of the Quality Indicator process. Knowledge of the OBRA regulations and Minimum Data Set. • Knowledge of the care planning process. • Experience with MDS 3.0, preferred.
