The Home Infusion Nurse Coordinator teaches patients, care givers and home health care nurses to administer infusion therapy in the home and makes recommendations regarding safety to MD and inpatient Case manager. The HINC will coordinate with health care team members to assure timely progression of patients across the continuum of care and coordinates with home health care professionals to assure all involved are prepared to participate in home infusion therapy for the patient. The HINC maintains contact with patient, care givers and Health Care Professionals through assessments via phone weekly or as needed. HINC supports quality improvement through evaluation of variances to identified protocols and maintains quality data systems, event reports and identifies and participates in quality projects and maintains inventory particular to patient needs. ***See Addendum for home visiting nurse*** EDUCATION: Current RN licensure or compact licensure recognized by the State of Vermont is required. ADN required, BSN preferred. Caring for and maintaining vascular access devices and clinical interventions for PICC issues: including clearance of occluded catheters, dressing protocols for skin issues, dressing assessment and management.
