Family First Healthcare Services in Florida is seeking an experienced Home Healthcare Clinical Manager, Registered Nurse (up to 20 hours per week) who is highly capable of building relationships to coordinate patient care transitions between different facilities and our agency. Our Clinical Manager, Registered Nurse, will work in close concert with and under the leadership of our Head of Clinical Operations . This Nurse lead will also play a lead role in overseeing daily operations, managing staff, ensuring high-quality patient care, and maintaining compliance with regulations . Other key accountabilities can include supervising nurses and support staff and developing and overseeing patient care plans. Our Home Health Registered Nurse (RN) lead provides medical care to patients in their own homes, which includes administering medications, performing wound care, and monitoring vital signs. Key responsibilities and duties · Patient assessment : Conduct head-to-toe physical assessments, monitor vital signs, and evaluate the patient's progress and condition. · Care plan development and management : Create and update individualized care plans based on physician orders, physical assessments, and the patient's home environment. · Medical treatment: Administer prescribed medications and treatments, perform wound care, and manage medical equipment like IVs or catheters. · Patient and family education : Instruct patients and their families on how to manage their condition, administer medication, and follow care plans effectively. · Coordination of care : Collaborate with physicians, social workers, physical therapists, and other healthcare providers to ensure a cohesive and comprehensive care plan. · Documentation : Maintain accurate and detailed records of patient status, treatments, and progress, which is often necessary for reimbursement from insurance or government programs. · Oversee the delivery of quality care, supervising clinical personnel and ensuring compliance with agency standards and healthcare regulations. · Lead and inspire clinical teams and clinician retention. Operate in a dynamic environment, balancing administrative responsibilities with field-based interactions. Referral Management · Receive, review, and process referrals from hospitals, physicians, facilities, and community partners. Verify completeness of referral documentation (face-to-face, orders, insurance, demographics, diagnosis codes). Communicate promptly with referral sources to obtain missing information or clarification. Patient Eligibility & Verification · Confirm patient eligibility and coverage with payers (Medicare, Medicaid, Managed Care, Commercial Insurance). Validate primary care physician and attending provider credentials and NPI. Confirm patient location, service area, and insurance plan network participation. Coordination & Communication · Collaborate with clinical intake nurses, scheduling, and field staff for start-of-care assignments. Communicate admission readiness and patient details to clinicians and case managers. Compliance & Documentation · Confirm that physician orders, F2F (Face-to-Face), and consent forms are complete before admission. Tracking & Reporting · Maintain referral logs and update intake tracking dashboards daily. Monitor conversion rates (referrals → admissions) and report intake metrics to management. · Staff and operations management: o Recruit, train, and supervise nursing and support staff. o Create work schedules and ensure adequate staffing levels. o Oversee daily operations to ensure a safe and efficient environment. · Patient care oversight: o Develop and approve patient care plans in collaboration with physicians and patients. o Conduct clinical assessments and ensure quality of care is met. o Act as a liaison between patients, families, and the clinical team. · Administrative and compliance duties: o Ensure compliance with all state and federal healthcare regulations. o Monitor performance metrics and quality of care standards. Nurse Manager I

Overview In accordance with the mission, vision, and values of the EVMS Medical Group and the Department of Obstetrics and Gynecology, the Hofheimer Hall (HH) Generalist Nurse Manager functions under the direction of the Director of Nursing and in collaboration with the Generalist Division Director, HH Generalist Practice Manager, and Department Administrator to provide clinical leadership and operational oversight for the Generalist Obstetrics and Gynecology practice. The Nurse Manager is responsible for the day-to-day management of clinical operations, ensuring the delivery of safe, efficient, high-quality, patient-centered care while overseeing clinical workflows, staffing, and regulatory compliance. This position provides direct leadership and supervision to the clinical team, fostering a culture of accountability, collaboration, and continuous improvement. The Nurse Manager partners with providers and administrative leadership to optimize clinical operations, support quality improvement initiatives, enhance the patient experience, and ensure compliance with organizational policies and evidence-based clinical practices. The incumbent also oversees staff recruitment, onboarding, training, performance management, and professional development to maintain a highly engaged and effective clinical team. Provides direct patient care, as needed, to support clinical operations, ensure seamless clinic workflow, and maintain continuity of high quality patient care. Travel will be required to all office locations and throughout all divisions to meet the department’s demands. Responsibilities Clinical Leadership and Operations (35%) Provides clinical leadership and operational oversight for the HH Generalist Obstetrics and Gynecology practice in collaboration with the HH Generalist Practice Manager and Generalist Division Director. Oversees daily clinical operations, ensuring efficient workflows, appropriate staffing, and delivery of safe, high-quality, patient-centered care. Develops, implements, and evaluates clinical workflows, processes, and protocols to improve operational efficiency, patient access, and the overall patient experience. Ensures smooth clinic flow across all clinical areas and proactively identifies opportunities for process improvement. Partners with providers and administrative leadership to support departmental goals, quality initiatives, and organizational priorities. Clinical Staff Management, Leadership & Development (35%) Provides direct supervision and leadership for HH Generalist clinical staff, including RNs, LPNs, and Medical Assistants. Manages clinical staff scheduling, assignments, and resource allocation to maintain appropriate staffing levels and support operational needs. Leads recruitment, onboarding, orientation, training, competency validation, ongoing education, coaching, and professional development of clinical team members. Conducts performance evaluations and addresses performance concerns in collaboration with the HH Generalist Practice Manager, Division Director and Human Resources. Promotes a culture of accountability, teamwork, professionalism, and exceptional customer service. Quality, Patient Safety & Regulatory Compliance (15%) Supports EVMS Medical Group quality standards and patient safety initiatives related to clinical operations and direct patient care. Implements and monitors clinical practices to ensure compliance with organizational policies, evidence-based standards, OSHA, CLIA, infection prevention requirements, and applicable regulatory guidelines. Ensures appropriate clinical documentation practices and monitors electronic health record documentation for accuracy, completeness, and compliance with medical record standards. Maintains patient confidentiality and ensures adherence to HIPAA and all EVMS/EVMS Medical Group privacy policies. Supports emergency response processes and assists providers with patient care needs as appropriate. Resource Management & Clinical Support (15%) Maintains appropriate levels of medical, surgical, and pharmaceutical supplies and ensures clinical areas are safe, organized, and prepared for patient care. Provides direct patient care support as needed, including patient triage, medication education, assisting providers with examinations and procedures, and addressing patient care concerns. Maintains professional competency through required training, continuing education, meetings, and organizational initiatives. Qualifications Minimum Qualifications and Skills Licensed Virginia Register Nurse (RN) (or Licensed Practical Nurse (LPN) with equivalent education and/or experience) is required. Three (3) years of supervisor/charge nurse experience is required. Preferred Qualifications Progressively more responsible positions in group practice management, in a large specialty group practice, strongly preferred. BSN preferred.
