Recognize and take appropriate action in situations that require immediate intervention. Manage Hub/facility staffing for assigned and next shift, if applicable. Collaborate with Nursing Supervision, Nursing Leadership, MCC’s and Order Organizer to ensure patient progression through appropriate allocation of resources. Proactively identify and remove barriers that may impede department or system-wide patient flow. Meet deadlines and exceed metrics identified for the Facility/ Unit through attention to detail and implementation of facility policies and Standard Operating Procedures (SOPs) What you’ll bring: Passion for Care Management 3+ Years acute care clinical RN experience required 2+ Years leadership experience preferred Prior RN experience in Utilization Management, Performance Improvement, Transition Planning and/or Care Management experience in an acute hospital or related managed care setting Education: Graduate of an accredited School of Nursing required Applicants currently enrolled in a BSN program may apply only if they are currently in a Care Manager role Bachelors of Science in Nursing (BSN) degree required Licensure: New York Registered Nurse (RN) License & Registration Certification: Basic Life Support (BLS) certification required; PRI certification required Salary Range USD $48.67 - USD $69.00 /Hr.
