Job Description Summary: An Infusion Nurse II is a licensed registered nurse who coordinates and provides intermediate direct patient care to patients in the home, or in an alternate infusion suite, to ensure patient safety with continuity and compliance under a physician’s plan of care. Works under structured supervision of the designated supervisor. Job Description: ​ Job Responsibilities (listed in order of importance and/or time spent) Initiates, develops, and implements intermediate nursing plan of care treatments, evaluating patient progress towards goals. Organizes and participates in the provision of direct patient care, performs treatments, administers medications, and educates patients and families/caregivers. Modifies plan of treatment in response to changing patient status or physician orders to achieve established or revised patient care goals. Assesses patient needs and physical status at each skilled visit through health data access and patient interview. Re-evaluates patient needs through physical reassessment, response to therapy, and supplemental physician orders. Obtains and clarifies physician orders for plan of treatment revisions, informs physician promptly of significant changes in patient's condition, and provides written summary to physician within supplemental order. Provides training and mentorship to Infusion Nurse I’s as needed to ensure patient safety and compliance. Provides effective and safe teaching using patient-centered care approach for patient and family to achieve independence with prescribed therapy and care needs through active participation per plan of treatment goals. Effectively and timely communicates with Option Care’s Clinical team, medical providers, patients, and families/caregivers to facilitate continuity of care. Prepares clinical documentation in real-time during visits and submits to the nursing department to comply with established timelines for billing optimization. Completes all documentation legibly and applies approved abbreviations and documentation error correction practices per Option Care’s policy. Coordinates discharge planning and prepares discharge summaries with patient instructions and thoroughly reports patient care needs, progress and goals when transferring care. Demonstrates compliance with agency operations, Option Care’s policies and procedures, professional standards, local, state, federal regulations/guidelines, and accreditation standards. Maximizes work efficiency through the use of computers and other automation technologies to validate plan of treatment orders, communicates patient care provided, and follows assigned visit schedule. Responds promptly and appropriately to patient requests. Initiates emergency procedures as necessary. Accepts accountability for own practice through ethical and professional conduct. Follows established programs and practice within policies and procedures reflective of Option Care’s mission, values, and objectives. Observes legal and ethical guidelines for safeguarding the confidentiality of patient and proprietary Option Care information including adherence to HIPAA regulations. Speaks knowledgeably about Option Care’s scope of services and effectively instructs patients about related financial obligations for care and service charges. Attends and completes required training modules, in-services, and continuing education to maintain competency and professional licensure for demonstrated knowledge regarding the care and management of patients in the home and/or alternate care settings. Effectively provides oversight and coordination of paraprofessionals in the home setting and may act as the Supervisor in the absence of the Supervisor or Nurse Manager as applicable in accordance with state and federal regulations. Participates in multidisciplinary team conferences and provides precepting, training, and mentoring to other nurses for orientation, and onboarding and supervisory activities as assigned. Participates in nursing department on-call responsibilities. Performs other related duties as directed by supervisor. If applicable: Inserts PICC/Midlines per physician order. Troubleshoots/declots obstructed central line catheters. Monitors PICC/Midline and educates on prevention of line infection and routine care. PRINCIPAL ACCOUNTABILITIES : · Understand and adhere to information and precautions contained in individual client’s Care Plan. · Assist the client with personal care activities including: bathing, skin care, back rub, hair care, nail care, dressing and undressing, feeding, oral hygiene, shaving, grooming and bedtime care. · Assist the client to the bathroom or in use of urinal or bedpan. Keep incontinent clients clean and dry. · Assist the client with client's self-administration of medications. · Assist the client with personal communication skills, as needed. · Prepare appropriate documentation of the client care or service(s) furnished. · Report all client and employee incidents/accidents to supervisor immediately. · Assist the client with supervising exercise, ambulation and transfer activities. Be aware of proper body mechanics. · Ensure maintenance of a clean, safe, comfortable and healthy environment for the client. · Observe safety precautions, including: wiping up wet floors, spills and other falling hazards immediately. Also, report safety hazards, frayed electrical cords, unsecured handrails, malfunctioning smoke and carbon monoxide detectors/alarms, defective equipment or environmental hazards to supervisor on the same day of observation. · Engage in respectful social interaction with client, including friendly conversation and empathic support, as well as, respecting client’s privacy and property. · Exhibit positive attitude and behavior and demonstrate respect for employees and clients and maintain absolute confidentiality of all information pertaining to clients, clients' families and employees. · Respond and attend to client’s requests promptly. · Communicate effectively with all members of the team. · Maintain proper hand washing techniques. · Participate in required in-service training programs to meet regulatory requirements. · Understand the importance of seeking assistance, as needed, from your supervisor and/or others; demonstrate capability and dependability in following instructions. · Understand that regular, consistent attendance is necessary to serve clients. · Accept and fulfill assignments with the Agency; exercise judgment in accepting assignments. · Perform related duties and responsibilities as deemed appropriate by the management team. Medical Assistant- Canyon Medical Center

The Medical Assistant provides both administrative and clinical support to ensure efficient primary care delivery while fostering a smooth, compassionate, and patient-focused experience. Full Time/Benefits Eligible Monday- Friday 8:00am-4:30pm Reynoldsburg, OH ESSENTIAL FUNCTIONS AND RESPONSIBILITIES: Work closely with the clinical care team to deliver the best care for our patients. Foster a climate allowing for direct communication between the care team, patient/caregiver and appropriate care providers to optimize outcomes. Collaborate with other care team members to address gaps in care (chart prep). Promote and facilitate improved clinical outcomes and patient satisfaction, as well as efficient use of resources. Act as a patient advocate to ensure highest quality of care for our patients. Set-up medical equipment and perform related tests as applicable, administer injections, and perform routine specimen collection and point of care tests. Responsible for vaccine administration as directed. Interview patients regarding medical concerns and obtain medical history. Measure and record vital signs, such as pulse rate, temperature, blood pressure, height and weight in patients’ EHR. Responsible for blood draws and specimen processing. Assist in performing routine medical procedures as requested. Prepare patients and treatment rooms for exams. Drape patients with covering and positions instruments and equipment. Review physician orders and lab requests and follows up as needed. Call in prescriptions/order refills as approved and directed. Communicate test results to patients as directed. Assist in patient education with related educational tools, periodicals, flyers, and handouts. Assist with applicable forms and patient paperwork (not limited to processing of referrals, prior authorizations or pre-certifications, etc.). Respond to patient phone calls, as appropriate. Maintain a clean and well-organized work area. Maintain patient confidentiality in alignment with HIPAA regulation. Attend meetings and participate in classroom activities as needed.
