Inpatient Neuroscience Intermediate Medical Surgical Telemetry unit is composed of 32 beds. Our patient population includes stroke, TBI, spinal and brain surgery, migraines, seizures, hydrocephalus, multiple sclerosis, myasthenia gravis, and infectious diseases processes such as meningitis. Intermediate Neurology populations includes patients that are post TNK and/or post thrombectomy, blood pressure and heart rate drips for stroke patient population, and cerebral spinal fluid drains. Essential Duties and Responsibilities: Administers medication and treatments as assigned within the scope of practice. Passes morning, afternoon and evening bedtime and overnight medications scheduled and as needed, staying with each resident until the medication is consumed, then charts appropriately. Maintains upkeep and cleanliness of medication carts and medication room, wipes down top of medication cart daily. Keeps the medication carts appropriately stocked. Responsible for recording and/ or restocking of medications from pharmacy no later than 7 days before medication runs out. Responsible for locking and storing narcotics (according to protocol) until turned over to pharmacy. Maintains accurate, complete and confidential resident medication and care records, according to established procedure and charts any unusual incident or information in resident Progress Notes and in staff Communication Log. Documents any and all changes in Communication Log along with giving a brief report to on-coming Medication Aide. In collaboration with the clinical team, verify allergies, NPO status, renal function, and other relevant clinical factors. Performs patient identification, procedure verification, and timeout processes in accordance with institutional policy. Perform sheath pulls and vascular access management as needed. Department Support Assist with managing supplies and par levels, restocking room, tracking expiration dates, rotating stock, and receiving, unpacking, labeling, and shelving incoming inventory. Assist with room turnover efficiency by assisting with post procedure cleanup such as sharps and biohazard materials disposal according to policy and preparing the room for the next case (restocking, setting up basic items, checking lead availability). Assist with ensuring devices are cleaned, charged, and stored properly (e.g., ultrasound, IVUS, etc.). ESSENTIAL DUTIES AND RESPONSIBILITIES: Understand a client’s care plan and adequately implement it. Assist clients according to their care plan which may include: companionship, meal preparation and service, light housekeeping, transportation, shopping, medication reminders, bathing, body care, dressing, feeding, ambulation, positioning, transferring, toileting, and protective supervision. Notify the Care Manager of client concerns, changes in care or client condition and client emergencies. Notify the Care Manager and/or Staffing Coordinator of any safety concerns. Follow company Infection Control practices; notify the office if in need of appropriate Personal Protective Equipment (PPE) or supplies. Follow the ClearCare Telephony Instructions to clock in and clock out for all scheduled shifts and accurately update your task status for each client prior to clocking out and leaving the client’s home. For compensable worked hours that are not allocated to a specific client (such as training time), you must use and submit the Time Log and Time Certification for Non-Visit Time form for each period pay period. Conform to the company dress code as specified in the Employee Handbook. Report to work on-time and ready for the day. Complete ongoing training as required by client care needs and/or changes in company policies and procedures. Attend staff meetings. Adhere to all company policies and procedures as detailed in the Employee Handbook. ORGANIZATIONAL RELATIONSHIP: The caregiver is supervised by the Care Manager and receives scheduling direction from the Staffing Coordinator. PHYSICAL AND ENVIROMENTAL DEMANDS: Reposition and transfer clients, sustain weight of clients using proper transfer safety techniques and equipment (example: use of transfer belt while assisting client to transfer from bed to chair), and travel to client’s residence place of residence using a reliable means of transportation.
