In collaboration with the patient, family, and health care team (IDT), the registered nurse as case manager will develop, implement, and evaluate nursing care according to agency and system policies. Understands pathophysiologic process by taking appropriate nursing action in accordance with changes in patient condition. 13. Reports significant observed changes in patient condition to Administrative Staff and/or physician. 14. Documents patient status and care or services furnished in appropriate areas defined by the documentation policy and procedure. 15. Documents home health aide supervisory visits as per standard. 16. Performs CPR when directed by patient’s appropriate representative’s wishes. 17. Displays safe, efficient, and prudent use and maintenance of D.M.E. and medical supplies and adaptive equipment. Notify admin/doctor if safely issues R/T DME in home. 18. Demonstrates bag technique per policy and procedure. 19. Able to apply and maintain equipment according to the Policy and Procedure Manual. 20. Utilizes supplies effectively and to ensure cost containment. 21. Maintains integrity of the IV site while performing patient care. 22. Performs treatments/procedures according to specific standards (e.g., vital signs, patient weights, specimen collection, ostomy care [ileostomy, colostomy, urostomy/tracheotomy], catheterization [urethral, suprapubic], wound care, dressing changes, suctioning [oral, pharyngeal/nasal/trach], gastric tube [feedings and maintenance]. 23. Performs and monitors patient teaching according to the policy and procedures. 24. Coordinates care so that medications are administered as ordered by a physician and in compliance with the medication administration. 25. Coordinates the care team in order to facilitate a change in the level of care in compliance with the change in level of care standard. 26. Identifies the stages of death and dying. 27. Accurately and timely, completes plan of treatment and recertification as per Medicare guidelines. Completes OASIS SOC/ROC, OASIS Recertification, and Discharge Oasis per guidelines. In the event the location does not have an Admissions Manager, the Admissions Registered Nurse will receive direction from the Patient Care Administrator or program designee QUALIFICATIONS FOR THE ADMISSIONS REGISTERED NURSE 1. Graduate of a National League of Nursing accredited school of nursing. 2. Current licensure in State of CA and CPR certification. 3. Registered nurses shall have a minimum of one (1) year of experience as a professional nurse within the last three (3) years; OR have a baccalaureate degree in nursing from a program accredited by the National League of Nursing and a current RN license. Minimum of two (2) years’ experience, at least one of which is in the area of public health, home care, or hospice nursing is preferred. 4. Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist. Admissions Registered Nurse is expected to have strong presentation skills. 5. Prolonged or considerable walking or standing. Able to lift, position and/or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping bending, kneeling and/or crouching. 6. How you make a difference The Licensed Practical Nurse (LPN) plays a vital role in patient care by contributing to the nursing process of assessment, planning, implementation, and evaluation. Working under the supervision of RN(s), the LPN provides direction and guidance in patient education and activities that align with their education and demonstrated competencies. Key Responsibilities Under RN supervision, assesses patients' physical, psychological, and social dimensions in both the Health Care Unit and housing units, contributing to individualized treatment planning and consulting with RNs and staff as needed. Implements treatment programs directed by Health Care Practitioners, administering medications, including IV therapy (if certified), and performing diagnostic tests according to established protocols and procedures. Collaborates in controlled substance counts at the beginning of each shift, adhering to safety techniques, procedures, and standard precautions while responding to codes or health emergencies within standard guidelines. Assists in medical or minor surgical procedures, implements nutrition and therapeutic diet plans, and documents nursing encounters using SOAP charting, ensuring effective communication with nursing staff, physicians, and other relevant personnel. Attends mandatory staff meetings and training sessions, demonstrating a commitment to continuous professional development and adherence to policies and procedures in providing high-quality patient care. Qualifications & Requirements Education Graduate from an accredited school of nursing. Experience Prefer a minimum of one (1) year clinic experience Licenses/Certifications Have and maintain current licensure as a Licensed Practical/Licensed Vocational Nurse within the state of employment.
