Overview Helio Health is on a mission to treat and promote recovery from the effects of substance use, mental health disorders, and other behavioral healthcare issues. To support the mission of Helio Health, the Licensed Practical Nurse (LPN) will work in a variety of clinical settings providing medical and counseling services as a member of the multi-disciplinary treatment team. The Licensed Practical Nurse (LPN) will work on a Full-time basis out of Helio Health’s Rochester Comprehensive Outpatient Program. The schedule will be Tuesday- Friday, 5:30am-3:00pm and Saturdays 7:00am-11:00am. Pay Rate: $26.00- $28.00 per hour. In addition to Helio Health’s comprehensive benefits package, all Full-Time LPNs are eligible for a Sign-On incentive up to $4,000 as well as shift differential! Responsibilities Works in a variety of clinical settings providing medical and counseling services as a member of the multi-disciplinary treatment team. Work primarily with methadone dosing. Provide client services for medical issues. Monitors and documents clients’ medical status, signs and symptoms of withdrawal and reports to supervisor or medical director. Works with other medical staff for continuity of clients’ medical care. Communicates with clinical staff regarding clients’ medical concerns. Works directly with clients in providing medical services and interpreting medical information. Provides direct client care of medical assessment, implementation of medical orders, communicable and infectious disease reporting in accordance with the law. Provides public health and screening protocols. Provides and documents medical consultation, assessment, examination, and treatment planning and review consistency with agency policy and requirements Qualifications Must be a licensed practical nurse currently registered by the New York State Education Department. Experience and/or training in the treatment of substance abuse and/or substance dependency preferred. Methadone experience preferred. Basic computer skills required. Places in the research folders. c. Calculates correct dose of drug to be administered after determining the Body Surface Area (BSA). Subsequent dose modifications based on toxicity may be necessary and require recalculation. 2. Protocol Compliance Ensures correct return clinic appointment or hospital admission. Schedules required tests and procedures for follow-up. c. If patient being seen at outside facility, corresponds with outside physician to ensure that protocol is followed and that tests and procedures are performed. Obtains treatment records from hospital or physician and obtains outside laboratory results. d. Ensures that study toxicities are recorded and graded correctly and accurately. If Serious Adverse Events are experienced, follows FDA guidelines for prompt reporting within 24 hours. e. If a violation occurs, fills out Violation Report, has a hard copy signed, and submits to the QA supervisor. 3. Investigational Drugs Works with the Norris Research Pharmacist in maintaining accuracy of drug logs. Obtains and transport investigational drugs to LAG Medical Center from IDS Pharmacy (If LAG patient). -Promptly returns unused drug from LAC within 24 hours. Ensures patient compliance when taking oral investigational drugs. 4. Pharmacokinetics/blood draws Arranges admission to the Clinical Trials Unit (CTU) at University Hospital so that samples can be obtained For other research blood draws: At Norris, fills out specimen request slips and coordinates the drawing of specimens with the clinical lab, processes samples after blood is drawn, and stores at the appropriate temperature. At LAG, will also draw the blood sample, and give to specimen person to process. c. Supervises packing, transporting and shipping of research samples to sponsor in accordance with IATA and sponsor shipping guidelines. 5. Clinical Research Forms (CRFs) When appropriate, ensures that Quality of Life questionnaires are completed. Ensures that data needed by the Data Manager to complete CRFs are obtained and documented. May be responsible for completion of some CRFs 6. Follows CISO SOPs II. Patient Care Related A. Physician/Investigator Interaction 1. Acts as consultant to PI in accruing patients. C. Hospital Staff Interaction 1. Conducts in-service classes for nurses, pharmacists, and CTU personnel for new investigational drugs/protocols. Functions as the resource person for protocol study questions. Communicates any patient related problem or concern to staff nurses, social workers, and home health coordinators. 4. Works closely with pharmacists and hospital nurses to ensure that protocol agents are administered accurately and safely. Coordinates obtaining, processing, and transporting of research specimens with hospital lab, CTU and destination lab. If admission to the CTU is required, coordinates each admission with CTU staff; is responsible for completing standardized orders and obtaining PI signature. (10%) Demonstrates expanded rooming protocol by assisting the provider in addressing care gaps with the patient and appropriately activating standing orders. (10%) Accurately performs clinical transcription during the patient encounter by documenting elements of the clinical visit under direct supervision of the provider and assisting with computerized provider order entry under direct supervision of the provider. (10%) Uses a motivational interviewing approach to coach patients toward self-management, self-efficacy, and activation. Engages the patients to assess current state, collaboratively set goals, and review the after-visit summary. Instructs the patient on condition management, behavioral support, and addresses emotional demands of having a chronic condition. Reinforces the clinical care plan and follows up for ongoing activation. (10%) Performs registry and quality data review and conducts population outreach to close gaps in care. (10%) Oversees the identification of high risk patients and facilitates complex care management referrals. Works collaboratively with care team nurses to provide care coordination to rising risk patients. (10%) Reviews and assesses with care team members to achieve population health management goals, to improve patient access to care and to reduce avoidable emergency department visits. (10%) Cleans and sterilizes instruments per established standards. Ensures adequate inventory of medical supplies. Ensures all patient treatment areas are stocked with appropriate supplies using established inventory standards. (10%) Performs in-house testing following established standards and policies. QUALIFICATIONS: Minimum High School / GED or Completion of a Medical Assistant Program or LPN License Current State of PA LPN licensure OR Current multi-state licensure through the enhanced Nurse Licensure Compact (eNLC) OR Medical Assistant Certification. 2 years of Medical Assistant experience CPR certification. Hospice Admissions Registered Nurse RN

Job Title/Position: Admission RN Reports To: Clinical Director JOB DESCRIPTION SUMMARY The Admission RN plans, organizes and directs hospice care and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Patient Care 1. Completes an initial, comprehensive and ongoing comprehensive assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es). 2. Provides professional nursing care by utilizing all elements of the nursing process. 3. Assesses and evaluates patient’s status by: A. Writing and initiating plan of care B. Regularly re-evaluating patient and family/caregiver needs C. Participating in revising the plan of care as necessary 4. Initiates the plan of care and makes necessary revisions as patient status and needs change. 5. Uses health assessment data to determine nursing diagnosis. 6. Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process. 7. Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care. 8. Counsels the patient and family in meeting nursing and related needs. 9. Provides health care instructions to the patient as appropriate per assessment and plan. 10. Assists the patient with the activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optional comfort care. 11. Acts as Case Manager when assigned by Clinical Supervisor and assumes responsibility to coordinate patient care for assigned caseload. Communication 1. Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given. Records pain/symptom management changes/outcomes as appropriate. 2. Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required. 3. Communicates with community health related persons to coordinate the care plan. 4. Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers. Works in concert with the interdisciplinary group. 5. Provides and maintains a safe environment for the patient. 6. Assists the patient and family/caregiver and other team members in providing continuity of care. Additional Duties 1. Ensures that arrangements for equipment and other necessary items and services are available. 2. Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and inservice classes. 3. Fulfills the obligation of requested and/or accepted case assignments. 4. Actively participates in quality assessment performance improvement teams and activities.
